7.1 Collect and organize data and apply critical thinking to
interpret information rom clients and constituencies

• Student will work with residents to collect data and observe
person-in-environment
• Student will continue to observe supervisor and ask questions
when needed
Actions
• Engage with residents
• Volunteer in dining hall dinner/ activities
• Report changing behaviors of residents
7.2 Apply knowledge of human behavior and the social
environment, person-in-environment, and other
multidisciplinary theoretical frameworks in
interventions with clients and constituencies

• Student will get to know each clients background and heritage
• Student will study residents personalities and how they
interact with other residents and staff members
Actions
• Pay attention to each residents files/ chart book
• Be accessible to all patients MDS files and past/ present
assessments in genesis systems.
7.3 Develop mutually agreed-on intervention goals and objectives
based on the critical assessment of strengths, needs, and challenges
within clients and constituencies.

• Student will work with the supervisor to create individual
goals for each resident that promotes the most success for
them within the facility
• Student will work with supervisor to create the most
appropriate intervention suitable for the resident
Actions
• Attend home care meetings and discharge care plan meetings
• Continue doing quarterly assessments on patients
7.4 Select appropriate intervention strategies based on the
assessment, research knowledge, and value and preferences of
clients and constituencies.

• Student will research and observe client along with the
supervisor to insure appropriate intervention and assessment
strategies
Actions
• Work with supervisor to conduct care plans for patients
• Attend day meetings to receive update and changes on
patients
