Meaning

 Active voice subject to do something

 Passive voice subject to be done by something
Formula

S + P + O=Active Voice

S + P + O + tobe + V3 = Passive Voice

Example

 I write a letter (active voice)

 A letter written by me (passive voice)

I read a book (active voice)

S + P + O
A book is read by me
S + O + tobe + V3 + P
