• The patient is deemed unsafe to self-care and immobilize; previously her husband cared for her but after his
death, she is in a caring facility but desires to go home
• Currently nurses are focused on assisting her self-care

Assessment
A • Mrs. Lingi has emotional needs; her grief and lack of social support such as family prompts her to
misunderstand and ask to return to home
• She is currently unable to self-care independently

Recommendations
R • The nurse should use effective communication skills with direct and clear words to avoid misunderstanding
• Communication with the family to increase social support and her needs.
CONCLUSION

• The importance of culturally sensitive effective communication skills is established


• Both verbal and nonverbal communication delivers clarity, compassion, understanding and
avoid misunderstandings
• Therapeutic communication signifies the value of caring conversation with the patient and the
family as well in nursing to ensure well-being of patients and the nurse as well
• Both the ISBAR and SOAP note ensure patient safety communication

It is concluded that effective communication skills, reflection, cultural awareness and sensitivity
allows a nurse to provide safe and compassionate care and builds resilience in self.
