Awards/recognition received and record of source of
income/livelihood/occupation of parents from advisers;
classification of family; no. of siblings and birth order;
contact numbers; result of item analysis from the subject
teachers, BMI of students from MAPEH Teachers and
record of Truant students from the guidance office
2. Analyze the data gathered
3. From the analyzed data gathered identify
students’ needs in terms of

•Nutritional/Health Needs
•Behavioral/Psychological Needs
4. Identify teacher’s in-charge to the following program
