CLASS I COMPOUND AMALGAM
RESTORATIONS

• Occluso lingual (OL) restorations is used on maxillary molars when a


lingual/palatal fissure connects with the distal oblique fissure and
distal pit on the occlusal surface.

• Occluso facial (OF) restoration is on lower molars when the buccal


fissure extends onto the buccal surface and merges with the facial
fissure.

Cavity preparation
• The cavity is prepared in two parts

1. Occlusal part

2. Lingual part
Occlusal part
• No. 245 bur or small fissure
bur is used

• It should cut “off center” to


preserve the disto-palatal
cusp.
• the occlusal portion may
have a slight distal tilt to
conserve the dentin support
of the distal marginal ridge
Lingual part
• Position the same bur to cut
lingual portion
• the length of lingual cavity (outline form) dependent on the
length of fissure

• The mesio distal width of the lingual extension should not


exceed 1 mm, except to remove carious or undermined
enamel.
• Axial wall depth – 1 mm (0.2-
0.5 mm inside DEJ)

• Than slightly tilt the bur to


follow the contour

• Now use bur perpendicular to
axial wall (lingual surface) to
accentuate mesio axial and
disto axial line angle
• Keeping the bur perpendicular
to the tooth surface, round
the axio-pulpal line angle

Finalization of cavity preparation

• Remove any remaining caries

• Improve the retention if required (grooves in the mesio axial

and disto axial line angles)

• Smoothen the walls ( hand cutting instruments)


Occluso Buccal cavity prep in lower
molars
• Make a simple occlusal class I
cavity
and extend the outline form
through buccal fissure onto
facial surface
• Buccal portion of extension is
cut with side of bur
• Define line angles by directing
bur from the buccal aspect
• Change the inclination of bur
to establish correct axial wall
depth. And round the axio
pulpal line angle
