According to the Center Disease Control
(2009) there are approximately 100-200 suicide attempts for every completed suicide among young adults in ages 15-24 years old. In addition, suicide accounts for 12% of all deaths for the same age group annually. Why adolescents commit it? • Adolescence involves a transition from childhood to adulthood. • This time is referred to as the period of “STORM & STRESS”. (stressful events, situations, and or conditions that may increase the likelihood of suicide) • Cases of suicide in the Philippines is 2,053 • 7 Filipinos commit suicide everyday (2012) • About 1 person every 3.5 hours. • Age bracket with the highest number of suicide cases: 20-24 with a total of 342 cases. 279 males 63 females

both adopted and used all over the world including Asia. Established validity and reliability. It involves 6 personal problem questions, 19 suicide ideation questions and 11 Social Support questions. The respondents were asked to voluntarily answer the instrument after a thorough and clear instructions.
