C Section Audit
• It has been observed that caesarean section rate is increased in all our hospitals . In some hospitals c-section
deliveries out numbered normal deliveries. Andhra Pradesh stood next to Telangana in C-section rate. C-section
delivery could lead to both short term and longterm health effects on women and children.
• WHO-estimated that 6.2 million excess ie.,not medically indicated CSs are being performed every year. It is
difficult to fix an optimal CS rate since it varies between facilities.
• In one study conducted based on Modified Robsons criteria revealed that repeat C-section was major cause
followed by primi>37 weeks gestation .By reducing the number of primi C-section ,the C-section rate could be
reduced.
• WHO recommends C-section audit as one of the effective non -clinical intervention and use of evidence based
clinical practice guidelines, c-s audits and timely feedback to health care professionals to reduce Cs births. Taking
all into consideration a customised C-section audit format was prepared with guidelines and monthly reporting
format.

• Steps to Reduce Preventable C-Sections
• All pregnant woman in remote areas should be admitted in hospital10-15 days before EDD (Expected Date of
delivery)
• Periodical checkups i.e.., minimum 4 antenatal check ups are mandatory in which 2 checkups should be done by
the MO particularly in 9th month of pregnancy.
• Identification of high risk pregnancy and regular follow ups till the time of delivery.
• Meticulous treatments and follow ups for aneamia, hypertension, Gestational diabetes,and other medical
complications .
• Late admissions, when the precious time is lost and the patient in obstructed labour or foetal distress should be
avoided
• Try to avoid preventable C sections as far as possible.

SUMAN
Objectives-

1. To Provide high quality medical and emergency services and referrals.


2. To Create a responsive health care system.
3. To Use institutions and other community-based platforms like NGOs, SHGs, PRls and
elected representatives from community etc., To help spread awareness and mobilize
community and facilitate 100% reporting and review of [Maternal Deaths.
4. To Develop and establish a system of continuous client feedback and redressal of
grievances for maternity services create awareness, build capacities and motivation of all
service providers,
5. To Provide an interdepartmental platform for convergent-Action plans create a steering
committee for effective governance including formulating strategies, supervision and review
of the program for corrective actions.
6. Beneficiaries of the scheme are all pregnant women, aIl mothers up to 6 months post
delivery, sick new-born.
Services Guaranteed under SUMAN
• At least 4 ANC checkups (including one checkup during the 1st trimester, at least one checkup under
Pradhan Mantri Surakshit Matritva Abhiyan, lron Folic Acid supplementation, Tetanus TOXOID (TD)
injection and other components of comprehensive ANC package) and 6 homebased newborn care
visits.
• Comprehensive Abortion Care services in line with the MTP Act. Safe Motherhood booklet and
Mother and Child protection Card with service guarantee charter.
• Zero expense access to identification and management of complications.
• Free transport from home to health institutions (dial 104 ,108), assured referral services with scope of
reaching health facility within one hour . Free transport from home to health institutions (dial

case emergency and drop back from institution to home after due discharge(minimum 48 hrs.).
• Zero expense delivery and c-section facility in case of complications at public health facilities . Services
for Elimination of Mother to child Transmission (EMTCT) of HlV, HBV& Syphilis .
• Quality care by trained personnel (including Midwife/SBA)'
• Respectful care with privacy and dignity. choice for delayed cord clamping beyond five minutes/up to
delivery of the placenta.
• Early initiation and support for breastfeeding.
• Zero dose vaccination, Free and zero expense services for sick newborns and neonates .
• Birth registration certificates from healthcare facilities .
• Conditional Cash transfers direct benefit transfers under various central and state specific schemes.
• Postpartum family planning counseling .
• Time bound redressal of grievances including responsive call center/helpline, web Portal, etc.
SUMAN Program Implementation Action Points:

• Notification of 240 facilities to all the districts


• Compilation of facility checklist and services checklists for CEmONC (108), BEmONC (52) by APVVP
• Compilation of facility checklist and services checklists for Basic facilities(80)
• Issuing orders on grievance redressal mechanism – detailed instructions to all the districts
• Operationalizing 104 call center for grievance redressal
• Orientation of all call center staff on grievance redressal mechanism
• Conducting reviews for orientation and improvement in performance with respect to checklists
• Displaying the SUMAN name hoardings on the facilities with good performance.
• Translation of SUMAN posters and hoardings in to Telugu and Editable copies of posters and
hoarding to be received from Govt. of India
• List of SUMAN facilities to be shared and displayed at Panchayat, Zila Parishad, and WCD
Departments
• Printing and distribution of posters to all the SUMAN identified facilities

the issue with the concerned facility
• Conducting orientation meetings with PRIs (panchayatraj institutions), RKS(Rogi
kaltan samiti)/HWC (Health and wellness clinics), VHSNC(village health sanitation

• and nutrition committees), CSOs, NGOs, CBO (community-based Organization) and
SHGs(self help groups) for effective social mobilization.
• These community agencies to ensure:
• Organizing periodic meetings to generate awareness among beneficiaries and their
families.
• Raising awareness about the cashless care during pregnancy, childbirth and PN
period, including care of newborn and infants at public health facilities.
• Sensitizing the family on the importance of getting timely ANC checkups and
ensuring stress-free environment for a PW for healthy pregnancy outcomes.
• Ensuring 100% registration and social review (verbal autopsy) of maternal deaths
and addressing the social causes of maternal deaths
• In the event of having a problem in getting referral transport on time, the
Panchayat /community to come forward and facilitate it.
PC&PNDT Act Committee Meetings

• As per PC&PNDT Act :- Conduct frequent meetings of the Advisory Committee


to expedite the decisions regarding renewal, cancellation and suspension of
registration.

• As per minutes of the State Level Advisory Committee and Multi Member
Appropriate Authority meetings held on 19.02.2021: The District level Advisory
and Multi Member Appropriate Authority meetings shall be conducted once in
every 2months,same dates as of the state level committee meetings

Action points at PHC level
Medical Officer responsibilities-
• Identification high risk PW during first trimester and meticulous follow-up till the time of safe delivery.
• PHC/UHC will be responsible to tag the supervisor with the identified high-risk PW
• One supervisor should be tagged, and responsibility should be fixed to them to follow the identified high-
risk PW till the time of safe delivery.
• 4 ANC check-ups should be done in which 2 must be carried out by MO and the last check-up is
mandatorily carried out by the MO without fail.
• All high-risk women should be checked by the gynecologist during PMSMA day.
• Weekly monitoring of the supervisors, ANMs & ASHAs work for necessary action.

Role of ANM & ASHA –


• All high-risk should be examined by the doctor in the 104 vehicles and ECG facility must be utilized 100%
for these women.
• 100% enrollment in Anganwadi centers for special nutrition diet
• Extensive interpersonal IEC should be done regarding the facility provided towards transport of women
in 108 and feeder ambulances
• Awareness on health programs focusing on avoiding teenage pregnancies, usage of temporary methods
in spacing of births should be carried out by utilizing VHNDs & VHSNCs platform.
• IFA tablets to reach all the target women (Adolescent girls & Pregnant women)
• Albendazole tablets – 1 tablet after 1st trimester
• 360 Calcium tablets
• PMSMA –
– Tag 1 physician to 4-5 facilities and mapping to be carried out by ADMHOs & DPHNOs
• RCH portal registration –
– Monitoring of all PW registrations to ensure
• Matru Samman Dress –

• LR Registers – Ensure the newly sent registers approved by GOI to be filled regularly as per
the annual delivery load
