LOCK OUT DEVICES
• Lock out devices such as padlocks,
clamps and chains are used in
conjunction with danger and out of
service tags. The objective is to
ensure the isolation and lockout of
electrics, switches, valves and other
equipment. Under no circumstances
should any person remove another
person’s padlock/lockout device.

GENERAL REQUIREMENT
The following general requirements for isolation and tag/lockout shall apply:

All isolations shall be individual isolation/tag/lockouts. No group isolations are permitted.
Two or more isolations may be installed on the same piece of plant; however separate permits/isolations
shall be used.
All tag/lockout devices shall be uniform on all Century sites.
Isolation tags shall be identified by marking on them the authorised persons name, date and reason for
isolation.
Any moving parts, which could cause injury through free movement, or falling, even though disconnected
from energy sources, shall be physically restrained and tagged. Blocks, wedges, chains and lashings
may be used as appropriate.
This procedure does not apply to any work on equipment that can be and is rendered safe by unplugging
its cord, and where the cord is under exclusive control of the person performing the work, i.e. air
conditioners.

PROCEDURE FOR ISOLATION
Identify the equipment/plant to be isolated.
Apply “Permit to Work” system.
Confirm the effectiveness of the isolation (access the hazards).
Open appropriate valves on compressed air and/or hydraulic systems to
release stored pressure
(energy).
Isolate the plant/equipment using the danger tags and/or the safety
locks/clamps.
Test that the plant/equipment is isolated and de-energised.

DURING MAINTENANCE JOB / SERVICE JOB INSIDE MUD TANK, SOME EQUIPMENTS
NEED TO BE ISOLATED. STARTED FROM THE ALL VALVES NEED TO BE CLOSED (IF
NECESSARY), AGITATORS, TRANSFER PUMP UNIT THAT CAN BE TRANSFER MUD INTO
THE TANK. ALL THIS EQUIPMENT NEED TO BE LOCK OUT TAG OUT. MOSTLY ELECTRICAL
EQUIPMENT CAN BE ISOLATED BY SWITCHED OFF ON ENGINE ROOM SUCH AS
AGITATOR
