LOCKOUT/TAGOUT

(LOTO)
Introduction

Why LOTO (Lockout/Tagout) procedure?


To protect employees who must do service or
maintenance works on machines and/or
equipment and who could be injured by an
unexpected start-up or release of hazardous
energy.

Definitions
LOCKOUT
• Placing a lockout device on an energy-isolating device*, to
ensure the equipment it controls cannot be operated until the
lockout device is removed.
*A mechanical device that physically prevents the
transmission or release or energy (ex. circuit
breakers).

TAGOUT
• Placing a tagout device on an energy-isolating device, to
indicate that the energy-isolating device and the equipment it
controls cannot be operated until the tagout device is
removed.

The lock and tag are what protect


you from systems being re-
energized while you are working on
them.
If you do not install them, you are
NOT protected!

Lockout/Tagout

Most common hazards to workers are


due to:
• Electricity;
• Hydraulic pressure;
• compressed air;
• Gas;
• Steam;
• Other fluids.

Responsibilities
Unit managers/Data engineers: must enforce the use of lockout
and tagout devices when employees do service or maintenance
works and may be exposed to hazardous energy.

All employees: Employees who do service and maintenance


works must follow the lockout/tagout procedures described in
this document.

LOTO – Energy control

Before begin maintenance or service works:


1. Inform all affected employees of equipment shutdown.
2. Shut down equipment.
3. Isolate or block hazardous energy.
4. Remove any potential (stored) energy.
5. Lockout and/or tagout the energy sources.
6. Verify the equipment is isolated from hazardous energy and
de-energized.

Before Re-starting the equipment (when remove


Lockout or Tagout):
1. Remove tools and replace machine or equipment
components.
2. Inform co-workers about energy-control device removal.
3. Ensure all workers are clear of the work area.
4. Verify machine or equipment power controls are off or in a
neutral position.
5. Remove the lockout or tagout device.
6. Re-energize equipment.

Energized testing

When an energy-isolating device is locked and/or


tagged and it is necessary to test it:
1. Remove unnecessary tools and materials.
2. Ensure that all other employees are out of the area.
3. Remove locks or tags from energy isolating devices.
4. Proceed with test.
5. De-energize equipment and lockout or tagout energy-
isolating devices.
6. Operate equipment controls to verify that the equipment is
de-energized.

• Always apply LOTO to maintenance or service of
energized machinery.

• Never avoid the security features.

• Confirm if a ‘Permit To Work’ is required.

• Inform the persons working in the dangerous zone.

• Perform a Job Safety Analysis before starting the work, in
order to assess the potential hazards of the activity.

 Procedure involving more than one person:


Each person shall install his/her own personal lockout/tagout
device.

 Procedure involving more than one shift:


Advise (manually written and verbally) the person who will
work the following shift about your LOTO work and warn
him/her in case of existing risks; do not remove the devices
until the work is finished.
