Permit to Overload Subjects for
Grade 12
1.Endorsement from the Public
Schools District Supervisor.
2.Letter request to overload
subjects of the school head,
parent and student.
3.Curriculum content checklist
with comments and
recommendations of the school
registrar/school head.
4.Waiver of the teacher who will
handle the overload subject/s.
5.Class program of the school for
the current school year with the
corresponding teachers
handling the subjects.
6.Student class program with the
overload subjects and the
corresponding teachers
handling the subjects.
