STEP 1. Write A Brief Introduction

Step 2. Create a table (Table 1) to assist your task by listing:
the hazards and the risks by each task/work activity

• Remember that the Task/Work sentence must contain the ‘Verbs’ (kata kerja).
• Hazards are those that can cause harm to the body, environment etc: There may be more than one hazards in a task
• Risk are the kind of injuries cause by the harms: there may be more than one risk for each hazard

STEP 3. Identify and assigning the Probability/Likelihood VALUES 

1. In an appropriate table (Table 2), identify or


select the best list of probability or likelihood
for the hazard and risk as identified in the
Table 1.
2. Assigned an appropriate value for each of the
probability/likelihood.
3. Ensure that the number values are
compatible and meaningful (make sense!)
STEP 4. Identify and assigning the severity/consequences VALUES

1. In an appropriate table (Table 3),


identify/select the best list of severity or
consequences for the hazard and risk as
identified in the Table 1.
2. Assign an appropriate value for each of the
severity/consequences
3. Ensure that the number values and lists are
compatible and meaningful (make sense!)
STEP 5. Create a Risk Matrix Table

1. In an appropriate table (Table 4),


insert/transfer the identified number values
into the appropriate cells in the table;
• The probability/likelihood cells (in the row)
• The severity/consequences cells (in the column)

STEP 6. Generate the RISK VALUE by filling in each cell in the table

1. Generate the risk value:


– RISK = PROBABILITY VS. SEVERITY
– RISK = LIKELIHOOD VS. CONSEQUENCES
2. In a table (Table 5), summarise the risk values
into appropriate category.
3. Example; extreme risk, very risky, major risk,
medium risk, minor risk or no risk etc. etc
(use color legends for each).
STEP 7. Generate a HIRARC Form

1. Transfer the information from Table 1 to the appropriate columns and


cells on the HIRARC form (Table 6).
2. Suggest the type of controls reasonably for each hazard and risk
identified.
3. Risk Value should correspond to the that in the Risk Matrik (Table 4).

STEP 8. Categorized and Rank the Tasks

1. Create Table 7 to categorised and rank the


task in order of most risky to the less risk as
the risk value generated in Table 6 by placing
the most highest to the lowest (Use color
legend as your guide)
2. Conclude in a paragraph (below the Table 7)
a statement of your finding of which task or
activity that is of a concern on the type of
risk for this job.
STEP 9. Safe Operating Procedure (SOP)

1. Write a standard/safe operating/work procedure either is a list or


in a paragraph form following each task.
2. The numbers of procedures depend on the number of work
activity as listed in Table 1 and Table 6.
3. Combine the words or statements used in the cells for each of the
risks, hazards and controls information in Table 6 into a sentence
in the form of an instruction or a guide or order sentence.
4. This can be a good way of ensuring all the elements discovered
and recorded in the table are associated to one another as
precautions' and instructions of the SOP.
5. Ideally, it’s also good to have the pics/graphics associate to the
type controls for each of the task.

• List the references used in completing this


assignment including books, internets or
notes.
• Cite a minimum of 5 references of which 1 is
from a book and others are sources from the
internet.
