Purpose: To check effectiveness of handling emergency situation.

PART A: - Sequence Planned :-

); Identifying & reporting of emergency situation: Planed on Fire Drill at Berth No-2 Area . Site

Manager, Engineer & Supervisor will inform HSE Executive I Project manager about incident. After
on spot discussion, emergency will be declare.
); Respective communication and other information to all concerned regarding emergency

with respect to the situation for fire, personal injury, natural calamity : ERT (Emergency
Response Team) coordinator will communicate all above information and coordinate for post
action.
); All clear siren/ declaration: Whistle will be raised to declaration of Emergency.

Specify the action to be taken by emergency response team: Emergency response team

> coordinator should be informing all personnel to evacuate the area immediately.
); Assembly: All personnel should be assembled at designated Assembly

Point. Head count confirmation: - Head count should be done by

); Identifying & reporting of emergency situation: - On 17/07/2020 at around 15.35 hrs. One welder (S.

Ravikumar, San Marine) was performed welding works near combustible storage drums, suddenly
catches fire inside of the garbage storage drums which are placed near hot work area at SBW area.
One worker (R. Ramesh Chandra Rao, San Marine) working near fire located areas, he was immediately
taken fire extinguisher & sand bucket which was placed near hot work area and used to extinguish the

Other workmen immediately came to the same location. One worker informed to site supervisor Mr.
V Suresh & he was informed to Emergency Contact Number, First Aider, Mr. Alok Kumar (CPE
Engineer), Mr. Pritam Mukhe1jee (HSE In charge). Decision was taken to declare emergency and
whistle was raised for declared emergency at 15:45 hours. everybody stopped their activities
immediately and gathered near assembly point
Respective communication and other information to all concerned regarding emergency with
respect to the situation for fire, personal injury, natural calamity: Mr. V.Suresh was informed to
P&A (Admin), for vehicles.
Specify the action to be taken by emergency response team: Emergency response team was
reached the spot and send to Assembly point all the workmen within few minutes.
Assembly: All personnel were assembled at designated assembly point.
Head count confirmation: - Head count was taken at the assembly point and found total 14 Nos.,
No one was m1ssmg .
Situation normalization. All clear message from spot: Situation normal was described by ER
team.
Briefing all concerned about the mock drill result: Mock drill result was briefly described by HSE ln­
Charge
Resume the normal activity: After declare normal situation, all workmen were resumed their duty.

Sequences of Emergency Preparedness:-
Time Details

15:35 At SBW area, Welder (Mr. S. Ravikumar) started to welding works near combustible
materials storage drums.

15:37 Materials storage drums suddenly catches fire.

15:38 Co-workers & Site foreman noticed this incident, immediately informed to site supervisor.

15:39 Mr. R. Ramesh Chandra Rao (San Marine) immediately reached at same point and
extinguishers the fire immediately.

15:40 Others crew members immediately reached at same point and help to extinguishers the fire
completely.

15:43 Ambulance with first aider reached at spot location.

15:45 Site Manager was declared site emergency instructing to stop all activities at that location.

15:47 HSE In Charge was called workforces to evacuate the area and gathering at emergency
assembly point further rescue management.

15:50 HSE In Charge addressed to workforces regarding emergency situations and details of
rescue management system of our company.

15:58 Head count was done along with the staff and workers, as it was recorded in muster roll
and confirmed the presence of all the personnel at that location.

16:05 Emergency declared off by the Site Manager and workforces ere sent back to resume
w their normal job.

Observations:
../ Immediately one person reached for extinguish the fire.
../ Everybody was aware about whistle blowing .
../ They also know the communication system as everybody gathered near assembly point, when
the emergency arises.

Post rehearsal Emergency Preparedness
Meeting:-
All workforces including sub-contractor were assembled and briefed about the emergency and
preparedness of rescue management. The potential near miss may cause serious incident and follow up the
procedures of safe working practices .

Recommendations:-
Emergency contact numbers should be displayed at SBW barge repairing
areas. Fire Extinguishers usage training frequently organized.
