•Analyze and present data
during Preparatory Activities
and follow these 7 stages to
identify Priority Improvement
Areas (PIAs)
1. Set general objectives to address PIAs
2. Organize Project Team
3. Listen to the voice of the learners and
other stakeholders
4. Analyze School Processes
5. Select area of focus
6. Conduct root cause analysis
7. Present root causes to SPT
ASSESS PHASE
These are the guide
question in this phase
 What surfaced as the most pressing need/
problem/challenge?
(For indicator with 3 year data) What year has
the most alarming data as per need/
problem/challenges?
Did the school improved? Stagnate? worsen?
(based on various indicators)
What needs the most improvement?
Is there a significant difference between the
school performance and the division targets?
ASSESS PHASE
Compare the school
performance to the Division
Target using GAP ANALYSIS
TEMPLATE
ASSESS PHASE
Identify the PIA’s (are
problems/needs of the school
to be addressed using Annex 4
Identifying Priority
Improvement Areas.
ASSESS PHASE
These are the guide
question in this phase
• What surfaced as the most pressing need/
problem/ challenge?
• ( For indicators with 3 year data) what
year has the most alarming data as per
need/ problem/ challenge?
• Did the school improve? stagnate? Worsen
(Based on various indicators)
• What needs the most improvement?
• Is there a significant diffences between
the school performance and the division
targets.
ASSESS PHASE
Compare the school
performance to the Division
Targets using GAP ANALYSIS
TEMPLATE ( Annex 3 GAP
Analysis Template)
ASSESS PHASE
Identify the PIAs ( are
problems/ needs of the school
to be assdressed using Annnex
Improvement Plan.
ASSESS PHASE
SPT will get the scores and last
priority ang transfer to ANNes
5 planning worksheet as to
Access, Quality and
Govenrnance.
ASSESS PHASE
Compare the school
performance to the Division
Targets using GAP ANALYSIS
TEMPLATE ( Annex 3 GAP
Analysis Template)
ASSESS PHASE
SPT will get the scores and
identify the first and last priority
and transfer to Annex 5 Planning
Worksheet as to Access, Quality
and Governance
ASSESS PHASE
The general objective should be
SMART
The general objective has the
following components: Time Period,
Target, Indicator and Baseline.
 Ex. To reduce the drop-out rate from 4% to 1% in 3 years
To+verb + indicator + by HOW MUCH+ by WHEN
ASSESS PHASE
After the PIA has been identified,
ORGANIZE THE PROJECT TEAM.
There can be 1 PIA Project Team, but
1 Project Team can be assigned to one
or more PIA, depending upon the size
of the school.
LAC session can be aligned with PIA
involving Teaching- Learning process.

ASSESS PHASE

complete and accurate data .
Review the General objectives and
target
Go back to Annex 5 Planning
Worksheet and write the root
cause/s
PLAN PHASE
Check if the General objective/s is
aligned with the root cause/s
Conduct Brainstorming of Project Team
to address the root cause/s of PIAs
There can be more solutions to a root
cause
Accomplish Annex 9 project Work Plan
and Budget Matrix
PLAN PHASE
Be sure to accomplished the
following in the Plan Phase
 Root cause column in the Planning
Matrix
Project Work Plan and Budget Matrix
SIP with signature of SPT members
AIP Template
ACT PHASE
• Test the solution in a small population to reduce the
risk of failure and give room for necessary
adjustments.
• The solution:
Address the root cause
Is within the control of the school
Is sustainable
Is economical
Is supported by the process owner.
Should be appropriate to the context of the school.
ACT PHASE
• Be sure to to do the following in the ACT
PHASE

APPLICATION
How would you understand School
Improvement Plan based on your own
understanding?

What do you think is the most important

function of School Improvement Plan?
After two years in Modular Distance Learning,
students already back into face-to-face modality,
however in some school’s teachers and principals are
worried because of the misbehavior and bad attitude
of their students inside the campus and in addition
they are not performing well specially in literacy and
numeracy.
How would you become an instrument
to solve this issue?

How would you apply the three phases

of SIP regarding on this problem.
SIP CYCLE SUMMARY
1. Data Gathering
2. School- Community Planning Team
3. Priority Improvement Areas (PIAs)
4. Root Cause Analysis
5. Solution
6. Testing of solutions
7. Implementation
8. Monitoring
