Principle:
 Designed to prevent or reduce the possibility of workplace accidents,
incidents or unit failures that could harm workers, the environment and
the plant itself.
 Every related stakeholder in the permit to work system shall implement
the PTW procedure.

 Know Permit to Work system and process

 More skill related to safety aspect during a hazardous work is executed

 Each stakeholder understands their position and responsibilities in the


PTW system

 The PTW system is applied to prevent / minimize accident, incident or


plant failure

RESPONSIBILITY OF PERSON
1. AUTHORIZED LEADER (AL)
a. AL shall comply with the procedure when preparing, confirming,
issuing/re-issuing and closing PTW document.
b.AL shall ask permission to Shift Coordinator in charge for any
work/testing which could effect safety to people, operating plant,
environment impact.
c. AL shall inform to Shift Coordinator in charge after work/testing has
been completed.
d. AL issue work permits based on hazard, risk and people
competency.

RESPONSIBILITY OF PERSON
2. AUTHORIZED OPERATOR (AO)
a. AO shall comply with these procedure when carrying out isolation,
de-isolation, and restoration as instructed by AL.
b.AO shall check and confirm that integrity of isolation is fulfilled.
c. AO shall complete the Isolation Restoration Instruction (IRI) before
giving it to AL for confirming.

3. COMPETENT PERSON (CP)
a. CP shall comply with these procedures when carrying out work or
testing whether instructions are issued verbally or written.
b.CP shall use safe methods of work, safe means of access and Personal
Protective Equipment which is provided for their safety (Establishing
& Maintaining General Safety)
c. CP receive work permit (verbally or PTW doc), carry out the
work/testing, clear work/testing, transfer or suspend PTW document.
CP = Maintenance Supervisor, Technician

3. COMPETENT PERSON (CP)
d. Maintenance Supervisor may act as Competent Person &
Supervisor at the same time
e. Maintenance Supervisor briefs the Competent Person in charge of a
working party stated on PTW document
f. Record all names of working party people
g. Make sure that the Work Order (WO) clearly inform that the work
to be done is under working party
h. Cross check the integrity of isolation points
i. Inform Authorized Leader if the System safety hazard arise
j. Inform Maintenance Supervisor if General safety hazard arise

4. QUALITY PERSON (QP)
Quality Person is responsible to make a report and recommendations
for specific works: Confined Space, Working at Height, Hot Work,
Excavation and Radiography.

a. CP must inform shift coordinator and or US/Field Operator
b.After work finished, CP shall check there’s no tools left at site
c. CP can ask field operator for equipment identification
d.When doing de-isolation and restoration, AO have to applying
mechanical part first then electrical part
