Permit to Work-- What? Why? How?


What?
• A Permit to Work (PTW) is a formal documented system used to
control work which has been identified as potentially dangerous.
• Part of the System of providing a “safe system“ to work.
• It is a statement, that all hazards and risks have been eliminated from or
controlled in the work area.

Why?
• Formalized system to ensure consistency.
• TBM- Duty to protect our employees and contractors working on specific
tasks.

How?
• Give authorized permission to work on SARB Package-4 (ZIRKU) site.

No limits for issuing permits


Apply one Work Permit for One Activity
E.g. For temporary camp installation as Cold Work
Permit, This will cover all related activities such as
scaffolding, material handling etc.
If any related activity may produce any ignition sour
ce such as hot works, use of equipments, power to
ols etc., then a separate Hot Work Permit is required
.

Permit Attachments HDEC HSE

Following items to be attachment to Permit Application:-


– Permit Application.
– Method of Statement.
– Risk Assessment / TRA / ORA (Communicated to employees).
– Location Drawing.
– LOP for Lifting Activity.
– Isolation Certificate (when required).
• List of equipment and tools to be used.
• Third Party certificates of equipments and operators .
• Other requirements for the respective tasks.
• Work can start only when MS is approved.

It is required before the start of physical work.

PTW Concerns HDEC HSE

1. Late application of permits – Ensure application is done the day


before.
2. Incomplete documents – Ensure all documents are in hand prior to
application.
3. Expired documents – No expired documents will be accepted.
4. Unreadable documents – All documents to be legible and clear.
5. PA not available in the work site – PA to be at area of work on PTW
6. PA do not follow procedures – PA to follow work procedures and
ensure that Safety rules are applied as per RA.

Oxygen Deficiency HDEC HSE

19.5% To 23.5%
1. Humans can survive for weeks without food, days without
water, but only few minutes without air.
2. Air contains a mixture of gases but is composed primarily
of nitrogen (78%) and oxygen (20.9%).
3. When levels of oxygen are reduced below 19.5%, serious
health problem begins to occur very quickly.
4. Oxygen is consumed by a variety of chemical processes:
5. Aerobic bacterial growth. (consume oxygen to live)
6. Oxidation of rusting of metals
7. Combustion and displacement by other gases. (welding and
cutting.

ISSUING PROCEDURES HDEC HSE

PART I
Performing Authority (PA) prepares the PTW, 24 hours in advance with proper coordination with the
Issuing Authority (IA) and HSE Authority (HSE).
HSE will review the Risk Assessment associated with the description of the work and its surroundings.

PART II
Issuing Authority (IA) verifies and ensures that Section 2 requirements have been implemented taking into account the associat
ed activities stated in Section 1.
Confirms that isolations are in place, tagged and locked whatever is required.
To liaised with Authorized Electrician, Authorized Mechanic, and Authorized Gas Tester and confirms Section 3 in PTW when
Certificates are required.
To attach visual displays which include a structured display of permits separating them by area and by status and a simple grap
hic display of locations on activities being carried out.
IA signs the permit and issue

PART III
Approving Authority (AA) is the Site Area Manager.
AA shall foresee the whole PTW system in his area of responsibility.
AA shall clearly identify all persons responsible under the PTW including the authenticity of the
signatories.
AA shall approved the PTW when all the requirements under the PTW system including safety
precautions as per Risk Assessment..

PART V
PA, after getting all required signatories in Section 2 he will confirms his signatures on section 5.

PA to leave the Pink copy of the PTW to HSE.


PA to leave the Yellow Copy to the IA.
And the White copy (original) will be made available on site.

Performing Authority (Work Permit Receiver) HDEC HSE
Ensures that
The task is fully discussed with the PTW Issuing Authority before signing to accept the PTW.
Showing the work party the exact location and equipment to be worked on.
Conduct TBT/Start Briefing to be completed by Supervisor before start the work.
The workers understand;
the details of the work
any potential hazards
precautions to be taken
the work stays within the PTW limits (area, types of work, time)
PTW are displayed at the task location.
Issuing Authority is informed when work is suspended or completed.
Stop work, if conditions at the worksite change.
Handing back permit on completion or suspension of the work.

HDEC CRITICAL ACTIVITIES on ZIRKU HDEC HSE

Critical activities include but not limited to the following shall require stage 2
TRA and shall be approved by the COMPANY:

Module Transportation ( Loading and Unloading of Modules).

Hot Tapping.

Heavy lifting of columns.

Night Works.

Deep Excavations (depth with more than 1.2 meters is to be considered).

Others as assessed by the PTW Issuing Authority.


For road movement with heavy/abnormal loads, the ADMA P
TW will be used.

NIGHT WORKS HDEC HSE

HDEC will utilize night work dispensation for every night activity or
activities beyond the normal working hours which is 6H30Hrs – 18H30Hrs
after 18H30Hrs need to be separate night work permit required.
The following shall be considered during night works:

A Task Risk Assessment approved by the COMPANY. Six months.

Dispensation Request approved by COMPANY (ADMA OPCO).

Duration of dispensation request shall be of Three months.

Urgent or priority works shall be granted and issued Permit to


Work provided that all the necessary safety precautions are
adhered including night dispensation if required.

ACTIVITIES WITHOUT PTW HDEC HSE

Routine activities which does not require PTW are the following but not
limited to:
Site housekeeping and facility cleaning ( E.g. Toilets and rooms
cleaning).

Office and Camp Maintenance ( E.g. Electrical bulb replacement


carpentry works, touch up paintings, etc.)

Inside Workshop activities


Note: Risk Assessments or MS will be required.

Activities not requiring permits shall not relieves the Area Manager/Site
Superintendent or whoever authorized the work to carried out, his
responsibility to control the job effectively.
The Area Manager/Site Superintendent must ensure in Toolbox Talk to a
competent person /Sup’v to carry the job safely and that the work must not
be hazardous to either the employees who carries it out or to anyone else.

P.A.Responsibilities HDEC HSE

 Confirming that the work site is safe for the work to proceed.
 Continuously monitoring work activity.

 Ensures necessary Controls are implemented and remain in place.


 TBT to be given by the supervisors prior to start the job and to be
attached with permit.
 Post permit on site and kept in PTW box.
 Communicating status of work to the oncoming shift or handover.
 Remains in close proximity to the work site designated to him.
 Inspects area prior to work start and addresses any SAFETY
concerns.
 Complies with Safety Officer requests / instructions.
 Complies with “Safety Inspection Checklist”

General Permit Guidelines HDEC HSE

• PTW not issued until exact location of the task is visited.

• Permit needs to be submitted minimum 1 day before the approval.

• Three copies White –Site/yellow -concern discipline(IA)/Pink-HSE


Dept.

• Issuing Authority cannot sign as Work Permit Receiver.

• Simultaneous work must be considered.

• Anyone can stop work if conditions change without revisiting the


PTW.

• Conditions must be continuously monitored.

• PTW records must be kept until the end of the SARB4 Project.
