A contractor was cutting a live 10” High Pressure flare
header line with a grinding machine. The gas invaded the
zone and a portable H2S detector and a multi-gas
detector were activated. A gas cloud formed which could
have been easily ignited. The operation was not planned
in the shutdown and the contractor did not ask for
approval of this work.

Risk Assessment

Fatality – Romania, 16th May 2013


While manually lifting an almost empty 200 liter plastic barrel of
hydrofluoric acid (70% HF) onto a truck the cap of the barrel came off
and some quantities of HF spilled over the left arm of the workover
employee. Despite water wash treatment on site and hospital medical
treatments later on, due to lethal effect of high concentrated hydrofluoric
acid on the skin the injured person died.

During pipeline cleaning/flushing activities of Nawara 24"
pipeline (pre-hydrotest) a cleaning pig entailing a lot of sand and
mud, had stuck before reaching the receiver end. After opening
the flange of the pig door, waste muds, waters and the cleaning
pig was ejected to a distance of 15 meters. This sudden relief of
pressure due to release of a 2nd pig was unexpected. Luckily no
one was standing in the line of fire.

Fatality – Romania, 1st May 2013


A park operator left his working place and climbed through a fence of a
nearby transformer station, opened the door of a switch container,
climbed over and behind a metallic horizontal electrical grounding bar
at around one meter height and came into contact to 6 kV electric
current. The Production Coordinator observed the power brake in the
area and came to the park looking for the park operator and found him
finally in the nearby transformer station inside the switching station
(container). He died in hospital.

Working at height

LWDI - Abu Dhabi, 22nd July 2017


The rig crew was dismantling the mast, as part of demobilization phase.
The work to remove the mast leg involved working at height to install
shackles onto dedicated lifting points to enable a crane to remove the
structure and place it on the ground in preparation for transfer to the next
well site. The Derrickman asked his colleague to unclip his safety harness
so that he could return to the cherry picker basket. He then stood up on
the beam and started to walk the 10 m back along the beam. As he
approached the cherry picker basket, he lost his balance and fell.

Fatality - Pakistan, 20th August 2015


The Mitto 2DSeismic is being shot in mountainous area near Mehar where
the terrain is rugged. The terrain was classified to control access as
mitigation for the injury risk from falling or slipping, but actually should only
be accessed by mountaineers. A cable helper with little climber experience
fell approximately 6 meters from a rock path into a gorge and was fatally
injured. He was part of a crew assigned to recover the cable and
geophones from a seismic shoot.

HiPo - Yemen, 12th April 2014


During tank construction, scaffolding erection inside of the tank was
authorized by OMVY. The crane was positioned and rigged up beside the
tank; prior to lifting any live load the contractor lifting supervisor ordered
to turn the crane rotating top assembly 360° to ensure safe distance to
the tank. During this test run the crane rotating top assembly fell off the
crane chassis. Two people were injured.

In order to install a pumping head on a well, the work-over platform had
to be secured in vertical position on the rig’s back. This was done by
lifting the platform with a traveling block and a sling while two crew
members were pushing the platform to its final position. While pulling
extra weight on the sling to facilitate the manual push, the sling broke
and the platform fell uncontrolled. It first hit the BOP and then one of the
drillers hurting his back.

LWDI - Romania, 26th February 2018
The crane operator went to the locker room when he arrived at work,
he dressed his PPE and continued to the dispatch room. Due to bad
weather conditions the activities were reduced, he went back to the
locker room. Around 08:10, he returned to the Dispatch department.
He used a path which crosses the parking area, where the snow
had already been accumulated over the ice formed the day before.
He slipped on ice and fell down, injuring his left leg ankle severely.

The duty operator, after taking over the night shift from his
colleague, was inspecting the equipment. While walking out of the
pump station he was blinded by the neon lighting of the oil metering
skid and didn't observe the hot water channel in front was
unsecured. He stepped with his right leg in hot water suffering
severe burns at the ankle.

HiPo - Romania, 20th June 2013
Last minute change in production test program led to the
preparation of a temporary flow line. When the Xmas tree’s
bottom master valve was checked before running the slick
line, it was found unexpectedly closed. By opening it 2 turns
(out of 18 turns to open it completely) the pressure, which had
built up in the meantime underneath the valve by gas from the
reservoir was suddenly released through the flow-line. The
jet-effect flipped over the flow-line at the open tank, threw it
against the wooden fence and knocked the fence down.
Luckily nobody was injured. This incident could have been
easily prevented by the experienced drillers on site who
watched the production team doing all these improvisations
without stopping the work.

Transportation Road Safety

Fatality – Tunisia, 30th September 2015
The driver got to the left of the road at a slight bend, tried to steer back
sharply to the right before losing control of the car. The vehicle rolled over
several times before ending up upside down some meters away from the left
side of the road. Only the front right passenger was wearing a seat belt at the
time of the incident. The car was equipped with seat belts for all five
passengers. The airbags were not deployed. 2 people were killed, 3 people
suffered multiple injuries.

While transporting a new 7 tone Pumping Unit with a flatbed truck from a
Storehouse to a well for its installation, the pumping unit got unbalanced in a
curve. The strap that held the Pumping Unit and the wooden bridge of the
truck-platform was broken. The Pumping Unit fell of the truck in town. The
pump was severely damaged but luckily nobody injured.

The administration building of a bioremediation plant is supplied with
water by an underground 5 m3 plastic tank via a pumping system.
After a few months of operations the tank was leaking and repair was
ordered by the construction companies which designed and build it.
The repair work was executed by subcontractors. During this repair
work an explosion occurred when the materials used to repair the leak
(Poly Urethane foam and blowing agent) formed a flammable
atmosphere which got ignited as a light bulb in the tank was powered
on. Two contractors were severely injured.

Fatality – Pakistan, 7th April 2016


The service contractor was preparing to lift a surge tank to load it on
the trailer. The plan to turn it into its horizontal position was changed
because the truck had not yet arrived. It was therefore placed between
the catwalk and the rig, out of the way for other rig equipment to be
loaded. Its distance to the catwalk was around 60 cm. One person
went up the tank to put the slings in the crane hook. The IP was
standing nearby and was not visible to the crane operator. The banks
man was on the other side to give signal to the crane. The Surge Tank
was lifted off the ground and swung away from the crane towards the
catwalk. At that time, the IP was in the process of putting vertical lifting
slings onto the I-Beam of the Surge Tank´s transportation frame. He
got caught in between the surge tank and the catwalk and his chest
and head was injured badly. He died in hospital.

Fatality - Romania, 29th June 2016


The Mixed Mobile Team (MMT), consisting of two mechanics
checked well 512 Barbuncesti. They heard a strange noise coming
from the beam pumping unit which they tried to investigate. Despite
the procedure of shutting down the pumping unit and despite the
warning of his colleague one worker got close to the V-belt
transmission system while it was in operating mode. He was hit and
crushed by the rotating crank and counterweight from the Production
Unit.

During a man lifting operation on the jack up rig crane the crane boom
pointer indicator, with a weight of 2.5 kg, along with the 1” retaining
nut dropped 12 m to the main floor. The indicator was most likely
disassembled and greased during an inspection therefore the nut was
loosened. No secondary retention was foreseen. Luckily, no one was
present on the main deck when the pointer dropped.

Position yourself in a safe

Fatality - Romania, 6th December 2013
To conduct the wireline survey it was necessary to move a pulling unit as
close as possible to the tank. Just then it was necessary to transfer the unit
back into rig mode and the Driver proceeded with the steps to make such
transfer. Believing the transfer to be successful the Derrickman went to the
Drillers cabin, put the unit in gear and applied the throttle. The unit jumped
towards the tank hitting it. For an unknown reason the victim, a workover crew
member had entered into the pinch point between the truck and the tank and
sustaining fatal head injuries.

An employee was hit by a backhoe loader bucket during the rotation of the
backhoe arm. The backhoe loader was excavating during a Kaizen event at a
well. The employee was not seen by the operator and hit at full speed on the
shoulder causing him to fall to the ground suffering a head and face injury.

While lifting a 4” pipe using a back hoe loader at well 305 Bodaesti, the
pipe swung and hit a mechanic. He was pushed in the front wheel of the
equipment, getting hurt at the level of his thorax.

A man was killed when the 14-foot-deep trench in which he was working
collapsed and buried him beneath tons of soil and debris. He was
working on the construction of a Restoration Hardware store in
Manhattan. The project's contractor did not provide cave-in protection for
the trench or support or brace a section of undermined and unsupported
sidewalk to prevent it from collapsing into the trench.

An explosive atmosphere was formed during a welding
operation on a pipe in an excavation. The gas evaporated
from drained oil in an excavation on the opposite side of the
road. The explosive atmosphere was blown by the wind
towards the welder through the inside of the duct and ignited.
The deflagration caused severe burns to the welder. The
chief of crew standing next to the welder just a minute ago
would have been hit fully by the explosion wave and could
have been fatal injured.

In 1980 Morecraft was working the night shift at an oil refinery in New Jersey. When he went to
one of the manifolds in the refinery to change out a blank on a pipeline, a surge of chemicals
came gushing out. He ran out of the manifold but when he went past his truck, which he had left
on, it ignited. When the ambulance came, his arms were charcoal black and they were swelling
up and oozing, and he could feel his face doing the same. He did not wear any safety glasses
and his coverall sleeves were rolled up.

Austria, 10th July 2013


A third party truck driver contracted by a local farmer titled the
charging box of his vehicle and got in contact with the 20-kV
overhead line of OMV Austria. A short-circuit to ground occurred
and the truck caught fire. An OMV employee went to the location
in order to find out the reason for the circuit to ground and found
the driver lying next to the truck, out of the cabin, he brought him
out of the danger zone, called the emergency center and started
reanimation measures. Unfortunately, the truck driver suffered
fatal injuries due to electrocution and died on site.

An explosion and fire at a polyvinyl chloride (PVC) plant killed five
people and severely injured three others. The plant had 24 essentially
identical batch polyvinyl chloride reactors. At the end of a batch, the
reactor was purged of flammable and toxic vapors and cleaned with
water. The water was then emptied to open sewer drains on the floor
below the reactor. It is believed that the incident was initiated when an
operator intended to drain water from a reactor which had been
cleaned. The operator went to a reactor in reaction stage. The reaction
mixture, at an estimated pressure of 70 psig, contained flammable
vinyl chloride. The operator was not able to open the pneumatic valve
on the bottom of the reactor – there was a safety interlock which
prevented opening the valve while the reactor was under pressure. It
is believed that the operator connected a nearby air supply to the
valve and forced it to open, releasing reaction mixture into the
building. Flammable vapors from the released material ignited.

This bus crash is often called the worst drunk driving accident in
United States history. A pickup truck driven by an intoxicated man
crashed into a school bus carrying sixty-seven people. Twenty-
seven children and teenagers were killed and thirty-four were
injured.

An oil tanker crashed on a road in Pakistan and at least
153 people, many of whom had rushed to collect
leaking fuel after the tanker driver lost control and
veered of the road, were killed when it exploded,
apparently after someone lit a cigarette, officials said.
