Welcome to Alkhorayef Water and Power Technologies Company. It is
our goal that your employment with us will be safe, productive, and
enjoyable experience.
This handbook outlines the Health and Safety guidelines applicable to all
employees. These guidelines have been developed to comply with
standard safety principles conforming to engineering practices and to
help provide a safe and healthful working environment. Please read this
handbook accurately be fully aware of its contents. If you have any
questions, discuss them with your supervisor or with your project health
and safety representative. Naturally, procedures and guidelines alone do
not guarantee that accidents and injuries do not occur. Your
cooperation and commitment to safety are critical components to make
the workplace safe for you and your co-workers. Be consistently alert for
potentially hazardous situations and strive to perform every assigned
work task in a safe manner. Accidents can be avoided by using good
judgment, common sense, and adherence to the company’s Health and
Safety program. Always work safely and encourage those working with
you to do the same.

Accident Prevention:

The safety and health practices described in this booklet have been
developed in keeping with the company's commitment to prevent
accidents and injuries, and to provide a safe and healthful working
environment on projects. Effective implantation of, and consistent
compliance with, the requirements and
procedures described below are critical to the protection and wellbeing of
everyone on the job site, including our customers, vendors, a n d the
public. Accordingly, it is your responsibility to read and fully observe al of
these safety and health practices while working on this project.

Basic Safety and Health Practices:

1. The supervisor will review the safety and health requirements for each
job task with you prior to starting work.
2. Know how to do your job safely.
3. Know the safe operating procedures before using a piece of equipment.
4. Make sure the work tasks you perform will not endanger you or your
fellow workers.
5. Follow the company safety and health policies and procedures as well as
posted signs.
6. Wear Personal protective equipment (PP) when required.
7. Keep work areas clean and orderly and continually check them for
hazards.
8. Pay close attention during job training sessions.

Emergency and Evacuation Procedures
Being prepared for an emergency can mean the difference between living
and dying.
Understand your part. You are our most valuable resource.

Reporting an Emergency
when you report an emergency, give the following information:
• The nature of the emergency - fire, medical, police.
• Your name and phone number or location.
• The location of the emergency
• Answer all questions to the best of your ability and stay by the
phone/radio and wait for further instruction.

Evacuation Procedures
A work area may be evacuated for numerous reasons. Know your
evacuation route and sate assembly area.

During an evacuation, remember to Keep talking to a minimum. Move


quickly. Do not run. Follow the directions of trained workers, and do not
return to your work area until instructed to do so by authorized
personnel.

Handling Victims

If you are the first to arrive at the accident scene, do not move the victim
unless there is an imminent dangers (i.e., fire, explosion, chemical exposure,
live wires, falling objects.

Call for help from the appropriate rescue personnel.

Reassure the injured that the ambulance is on its way.

Trained medical personnel and first aid stations are available on all job sites.
Make yourself familiar with their locations.

Remember safety first.

Fire and/or smoke

When a fire or smoke is discovered, sound the alarm. Alert the workers closest
to the fire to move to the nearest point of evacuation or to the assembly area.

If there is heavy smoke, get down below the level of the smoke and crawl to
the nearest exit or safe assembly area.

Always check closed doors for heat before opening. Do not open fi door is hot!

If trained to use afire extinguisher on small fires, use the Pull, Aim, Squeeze
and Sweep (PASS) method depicted in the graphic on the following page.

First Aid and Injury Reporting

If you are injured or become ill during your course of employment, first aid
treatment will be provided. Qualified personnel are also available to refer
you for medical treatment if required. Report all injuries, no matter how
minor, to your supervisor immediately, then report to the project First Aid
facility. Treatment will be provided, and the incident will be recorded so that
there will be a record of your occupational injury or illness.
If you obtain medical treatment off the project for a work- related injury or
illness, you must notify your supervisor and First Aid at the start of your next
scheduled work shift.

Prior to returning to work after an occupational injury or illness, you must


present a medical clearance (return-to work-slip) from the attending
physician to the First Aid facility. If the doctor sends you back to work with
restrictions or any kind (modified duty), your restrictions will be evaluated by
the Safety & Health Department and your supervisor to see if there is a
regular work task you can safely perform.

Prescription medications shall not be taken on the job unless authorized in


writing by your personal physician and reviewed by project management and
the Safety & Health Department. A copy of this authorization shall be
maintained in your confidential medical file. Some tasks may require
employees to have a physical examination to determine their health
condition.

These examinations are required by law to protect the employee and fellow
workers.

Competent Person
A “Competent Person” is one who has the experience and
knowledge needed to identify existing and predictable
hazards. Each project will designate a Competent Person
where the activity or task requires one. Here are some
examples of jobs that require a Competent Person:

Excavation and trenching work

Confined-space entry

Scaffold erection

Electrical installations / repair

Blasting

Underground Construction.

Substance Abuse

The Company expressly prohibits the possession, sale, use or


distribution of prohibited substances or related paraphernalia on
the Company's premises. Such acts are grounds for immediate
dismissal. While at work or performing on behalf of the company,
employees shall not have a measurable level of prohibited
substances or alcohol in their systems.

Pre-Task Planning (STARRT)

The purpose of pre-task planning, or STARRT (Safety Task Analysis


Risk Reduction Talk) is to analyse the safety and health aspects of
tasks that should be performed during the shist to develop a risk
mitigation plan and working safely. It is essential that everyone
participate in this process so that they understand the nature of
the hazards and the safety measures to be taken before work
begins. Make sure you have a pre-task plan in place before starting
your shift or new tasks.

Personal Protective Equipment
The proper selection and use of Personal Protective Equipment (PPE) is
an important element in the prevention of work injuries. However, PPE
can provide you with a level of protection against injury and illness only
when maintained and used correctly. You should be aware of all the PPE
requirements needed for your work site and assigned tasks and observe
postings at times. When PPE is specified for certain work tasks or
locations, its use is mandatory.
You should also wear the suitable clothing for the you are performing.
On the job site, the minimum attire is long pants (trousers) and T-shirt
with sleeves. Shorts, cutoffs, tank tops (vests), and mesh shirts do not
provide suitable protection and are therefore not allowed on company
projects.

Head Protection
Any person entering a work area with a risk of head injury must wear a
hard hat [American National Standards Institute (ANSI); BS ENJ]. The
hard hat must be specifically suited to the type of hazards present in the
work area, such as falling or flying objects, impact hazards, electrical
shock and/or burn hazards. The headband must be adjusted to fit snugly,
with the suspension adjustment placed on the back of your head. (Bump
caps or "cowboy style" hard hats are not allowed on projects.) Never
wear your hard hat backwards or rotate the inner headband so that the
hat can be worn backwards.
Welders require a special hard-hat configuration to accommodate the
welding shield.

Hard Hat Care and Inspection
always keep your hard hat in a dry, clean place. Never use paint, solvents,
cleaned hydrocarbon-type cleaners (such as M.E.K, thinner, gasoline, and
kerosene), glue, or pens on hardhat. These substances can cause serious
damage which may not be visible upon inspection.
Visually inspect daily the shell and suspension of your hard hat for
breakage, cracks, craze pattern, discoloration, chalky appearance, or any
other unusual condition. Also, inspect the shell for brittleness by flexing
the brim. Any of these conditions indicates a loss of protection from
impact and electrical conductivity. The equipment MUST be replaced
immediately.

Eye and face protection


In the workplace, some of the most common injuries are those to the eye
and face, resulting from flying particles, molten metal, chemicals, gases, or
radiant energy. Selecting and using proper eye and face protection can
prevent most of these injuries. Where eye protection is required, workers

glasses that fit over their corrective lenses. Ordinary prescription glasses,
unless made of safety glass, DO NOT provide adequate protection from
eye hazards encountered in the workplace. Ordinary lenses can shatter,
showering glass in the user's eye, and ordinary frames are not strong
enough to protect against impacts.

Selection and Use of Eye and Face Protection

Proper Safety glasses are American National Standards Institute (ANSI) or


BS EN approved and have frames and lenses that can resist impact from
flying fragments, objects,
large chips, particles, and dirt.

Safety Goggles fit tight to the face, surrounding the eye area to provide
extra protection from splashes, impacts, and sparks. They must be worn
when handling chemicals such
as acids and bases, irritating fumes and mists, and corrosive gases, when
there are severe hazards from flying particles. You can also wear goggles
over prescription glasses.

Face Shields are worn when maximum protection is needed for the face
and neck from flying particles and chemical splashes. Face shields are NOT
intended to provide fully
eye protection. Therefore, approved safety glasses or goggles MUST be
worn under a face shield.

Hand Protection

Gloves are made from various materials, each providing protection from a
specific type of hazard. For example, canvas and leather gloves offer
protection from sharp or abrasive objects. Rubber, latex, neoprene, and
nitrate gloves offer protection from specific chemicals. No single glove
offers protection from all chemicals. Your supervisor or project Safety
& Health representative can assist in selecting the right glove for the work
task.

Foot Protection
The protective shoe offers you protection from falling objects, puncture,
crushing, slipping, and electrical shock. Safety toe boots are required at
construction and construction sites. These are rubber boots that limit
chemical exposure and provide improved traction on slippery surfaces.

Fall Protection and Prevention


• The company's policy on fall protection requires you be tied off 100
percent of the time when you are exposed to a fall.
• Before using the fall protection equipment, you should have received
training on how to properly use the fall protection systems and
equipment as well as how to determine the safe anchorage points.
• You are required to carefully inspect all fall prevention equipment
before each use. If you find any damaged or corrosive components,
DO NOT use the equipment. Return it to your supervisor immediately.
• The personal fall arrest system will require the use of full body safety
harness with a shock absorbing lanyard with double-locking snap
locks.
• Personnel traveling or working in elevated areas, or adjacent surface
where a fall exposure exists, shall make use of secondary fall
protection by securing their safety lanyard on all lines to a structure,
lifeline, or approved fall-arresting device (emergency reel) capable of
supporting 5,000 pounds (2268 kilograms) per person.
• Always check for proper fit of your fall harness. It should fit snugly
with the flat side of the D-ring positioned between your shoulder
blades. Remember, a fall force will stretch your harness, so be sure it
fits smugly.

Shock-absorbing lanyards are designed to extend up to 3.5 feet (1.1
meters) Before stopping your fall. Also, if your anchor point is located
below your D-ring, this will increase the total distance of fall. it is always
preferable to anchor to a point above your D-ring.

Anchorage points must be capable of holding at least 5,000 pounds


(2268 kilograms) per person. NEVER use a conduit or ceiling grid as an
anchor point. Always make sure the locking mechanism on your snap
hooks is functioning properly. Never connect two snap hooks together
or double your lanyard back on itself.
100 percent fall protection is mandatory when working within 6 feet
(1.8 meters) of an unguarded edge of a building, platform, open-sided
floor, leading edge.

5. Materials used for sheeting, shoring, or bracing shall be in good
condition. Timbers must be sound, free of large or loose Knots, and

6. Employees working in bell-bottom caisson holes shall be protected by
a substantial casing that extends the full depth of the shaft. When
working in such holes, confined-space entry procedures must be
followed and a body harness must be worn and secured to a lifeline

attendant must be clear and audible above surrounding noise levels.

stairs, or ramps.
8. Trenches 4 feet (1.2 meters) or more in depth must have ladders

grade level.

10. Walkways, bridges, or ramps with standard guardrails shall be
provided where employees or equipment are permitted or required
to cross over excavations or trenches.
11. In locations where oxygen deficiencies or concentrations of
hazardous or explosive gases or dusts are possible, the atmosphere in
the excavation must be tested before anyone enters.

Hearing Conservation

It is critical to eliminate excessive noise exposure.


The best way to protect your hearing is to know the designated areas
and specific tasks that require hearing protection.

Several types of hearing protection devices are available such as


earplugs, hearing protectors, and earmuffs. You should select the type
that is most comfortable for you and make it a habit of wearing your
hearing protection properly.
Each type of hearing protection is slightly different, so you should
test the fit by cupping your hands over your ears, then releasing. If
you can hear a difference, you may not be wearing your hearing
protector correctly. Remove, and then fit again.

Always wear your hearing protection in designated areas or when


preforming designated work tasks.

Respiratory Protection
Respirators will be selected for specific applications by your project
Safety & Health representative. All respirator users must obtain
medical clearance, be fit-tested, and receive training in how to
wear a respirator. For specific information, consult your
project Safety &Health representative.

Working Over or Near Water


Workers must wear life jackets or buoyant work vests at all times
when working on, adjacent to, or over water.
One 30-inch-diameter (76.2-centimeter-diameter) ring buoy—
with not less than 90 feet (27.6 meters) of line attached shall be
immediately available for rescue operations. Rescue equipment,
including a boat or suitable watercraft, shall be available for
rescue operation.

Housekeeping
Housekeeping is a fundamental and necessary activity on company
projects and will be performed by every employee. Good housekeeping
practices lead to greater work efficiency, reduction in fire hazards,
cleaner work environments, better use o space, and improved morale.

• Keep walkways and doorways clear, unobstructed, and tree of


electrical cords. Boxes, and equipment at all times.
• Scrap materials are fire and accident hazards, so Keep them to a
minimum and dispose of them safely.
• Tools and materials must be placed where they will not create a
hazard for others. Spilled liquids can result in safety, health, or
environmental problem.
• Keep change areas clean. Do not let soiled clothes, food scraps, and
soft drink containers accumulate. Drinking cups, sandwich wrappers,
paper bags, and other trash must be placed in provided containers.
• Toilets, wash facilities, and potable water are provided for your
convenience and comfort.
• Remove all protruding nails, staples, screws, or other objects from
lumber or other building materials that present a hazards to
employees or vehicles.
• Trash containers will be located throughout work areas.

Material Handling and Storage
All materials shall be properly stacked and secured to prevent sliding,
falling, or collapse. Aisles, stairs, and passageways shall be kept clear for
the safe movement or employees and equipment, and to provide access in
emergencies.
Use proper lifting techniques when handling materials:
Establish good footing before attempting to lift.
Keep your back straight. Do not lean over.
Bend your knees; get down close to the load.
Lift gradually, using your legs. Do not jerk or twist.

• Get help for bulky or heavy loads.


• Whenever possible, try to use mechanical aids to reduce the amount of
lifting you’re required to do.
• When storing materials inside structures or buildings under
construction, you should place them:
• A minimum of 6 feet (1.8 meters) away from any hoist way or other
inside floor opening.
• A minimum of 10 feet (3 meters) away from any exterior wall that does
not rise higher than the top of the material stored.
• A minimum of 10 feet (3 meters) away from any exterior wall that
doesn't rise higher than the top ort h e material stored.
• Pipe, conduit, and bar stock should be stored in racks or stacked and
blocked to prevent movement.
• The quantity of materials stored on scaffolds or platforms must not
exceed the rated rapacity of the scaffold/platform. Materials must not
exceed the amount required for one day’s operation.
• Special storage requirements must be followed for reactive and/or
hazardous substance Incompatible materials such as oxidizers and fuels
shall not be stored together. Appropriate storage cabinets should be
provided for acids and flammable material.

Fire Prevention and Protection
All employees on the project should have basic knowledge of fire prevention procedures,
fire classifications, and the appropriate extinguishing agents. You are also expected to
know the location of fire-fighting equipment in your work area.

Fire Prevention Procedures


Keep your work area neat and free of hazards.
Only approved solvents are to be used for cleaning and degreasing. The use of gasoline
and similar flammable products for this purpose is strictly prohibited.
Where fire hazards are prevalent, check with your supervisor or the Safety & Health
Department office for proper precautions.
Flammable and combustible liquids shall be handled only in approved, properly labeled
safety containers.
You must place oily rags in covered metal containers.
Do not attempt any work involving a source of ignition near a pit, sewer, drain, manhole,
trench, or enclosed space where flammable gases may exist or may have been present.
Obtain a hot-work permit prior to starting any work task involving a source of ignition.
Open tires are strictly prohibited. A one-day supply of flammable /combustible liquids
(paints, etc.) is allowed inside a building in approved containers only.

Fire Classifications
Class A - Ordinary combustible materials such as wood, paper or cardboard: Water or dry
chemical powder is a good extinguishing agent
Class B - Flammable liquids and gases, such as gasoline, solvents, paint thinners, grease,
LPG, and acetylene: Dry chemical powders, foams, and CO2 extinguishers work best on
flammable liquid fires.
Class C - Fires in energized electrical equipment: CO2 and dry chemical powder
extinguishing agents can be used on electrical fires. Do not use water, as it is an electrical
conductor.
Class D - Fires incombustible metals: A special extinguishing agent is required

Hazards Communications
Materials such a s concrete products, paint, solvents, compressed
gases, and lubricants are considered "hazardous substances." To be
protected from hazardous substances at the job site, you must
understand and follow the written Hazard Communications Program.

Container Labeling
All hazardous materials must have product warning labels. If you come
across a product that is not in a labeled container, notify our Supervisor
immediately so that the material can be disposed of or labeled
properly. Never use unlabeled substances

Material safety data sheets
Material Safety Data Sheets (MSDS) are available for all the hazardous
materials on the job site. An MSDS contains specific information about the
hazardous properties of materials, and the protective measures you should
take to protect yourself.

Training
You must be properly trained in the handling and control of hazardous
materials prior to working with them.

Flammable and Combustible Liquids


1. When handling chemicals, it is especially important to use appropriate
personal protective equipment. See your supervisor or project safety
and health representative for appropriate equipment.
2. Chemical spills should only be cleaned up by properly trained
personnel.

containers

Power and Hand Tools
General
Tools shall be inspected daily to ensure that they are in proper working
order. Damaged or defective tools must be tagged ”Out of Service" and
immediately returned to the tool room for repair. Power saws, grinders, and
other power tools shall have proper guards in place at all times. Unplug
electrical and disconnect pneumatic tools before performing maintenance
or blade/bit changes. tools must never be hoisted, lowered, or carried by
the cord.
To prevent employees from tripping and falling, cords, leads, and hoses
must:
• Be placed out of walkways, and off stairs or ladders.
• Be secured a minimum of 7 feet (2.1 meters) above walkways, runways,
and ramps. All grinding wheels, wire brushes, and flapper wheels must be
rated for the grinder on which they will be used.

Electrical Tool

All portable electric tool shall be grounded (earthed), except approved,


double-insulated tools. All power tools shall be 110 V. Don't use electrical
tools in wet or damp locations.
Pneumatic Tools

An approved safety check valve (excess flow valve) must be installed at the
manifold outlet of each supply line for handheld pneumatic tools.
All pneumatic hose connections shall be fastened securely.

Safety clips or retainers must be installed on all pneumatic tools to prevent


the accidental expulsion of the tool from the barrel.
All bull hoses shall b e secured with a safety chain or equivalent.

Fuel-powered Tools

All fuel-powered tools must b e shut down prior to being refueled.

Smoking is prohibited during refueling operations. Other nearby sources of


ignition, such as burning and welding, must also be halted during refueling
operations.
Fuel-powered tools shall not be used inside a building or excavation
without adequate ventilation

Powder-Actuated Tools

Only employees who have been trained and certified are permitted
to use powder-actuated tools. Certification cards must be carried
by employees when using powder-actuated tools.

Follow all manufacturer's instructions when using powder-


actuated tools. Never try to release a loaded tool that has jammed
in the firing position.

Never point the tool at anyone. Check the opposite side of the
work area prior to discharging the tool. Always wear adequate eye
protection when using these tools.

Portable Ladders

Ladder safety begins with the selection of the proper ladder for the
job and includes inspection, setup, proper climbing and standing
procedures, proper use, care, and storage.

General Safety Rules for All Ladders

Ladder Selection - Be sure the ladder being used has the proper capacity rating to carry the
combined weight of the user. Use the proper size ladder for the job. Select a ladder that will put
you at the appropriate working height without overreaching or standing on the top rungs of the
ladder. (Note: The use of metal ladders is prohibited near live electrical circuits.)

Ladder Inspection - Always check a ladder before using it. Inspect wood ladders for cracks and
splits in the wood. Inspect all fiberglass ladders prior to use.

Ladder Setup - All ladders shall be secured at the top to prevent ladder movement. Prior to
setting up a ladder, survey the area overhead for electrical wires and other impediments. Set up
the ladder in a 4 to 1(vertical to horizontal) ratio. Place ladder feet firmly and evenly on the
ground or floor. Make sure the ladder is straight and secure before climbing it. The top should
rest evenly against a flat, firm surface. Do not allow ladders to lean sideways.

Ladder Use - Keep the steps and rungs of ladders clean. Also clean your shoes before climbing a
ladder. Never carry heavy or bulky loads up a ladder. Climb up first, and then pull up the
material with a rope. Do not overreach from a ladder. Never push or pull anything sideways
while on a ladder. When a ladder is used for access to an upper landing surface, it must extend
three rungs or at least 3 feet (0.9 meter)- above the landing surface. Always raise an extension
ladder from the ground. Check for stability and support before climbing. Never use a damaged
ladder. Tag it "Detective" and report it to your supervisor so that it may be removed from
service.

Scaffolding
Scaffolds or work platforms shall only be erected, moved, dismantled, or
altered under the supervision and direction of a Competent Person.
(See page 8 for definition of a competent person.)
Scaffolds shall be inspected by a Competent Person before each work
shift.

• Guardrails, midrails, and toe boards shall be installed on all open sides
of scaffolds. Scaffold planks must be at least 2-x 10-inch (5.1- × 25.4-
centimeter ) full- thickness lumber, scaffold-grade, or the equivalent.
• Scaffold planks shall be cleated or secured and must extend over the
end supports by at least 6 inches (1 5.2 centimeters) but not more than
12 inches (30.5 centimeters).
• All scaffolds should be fully planked and constructed with a safety
factor of four (4) times the maximum intended load.
• All scaffold components shall be visually inspected before each use.
Damaged scaffold components must be removed from service or
repaired immediately.
• Access ladders shall be provided for each scaffold. Climbing the end
frames is prohibited unless their design incorporates an approved
ladder.
• Adequate mudsills or other rigid footing, capable of supporting the
maximum intended shall be provided.

• Scaffold shall be secured to the building or structure as specified by
design.
• Scaffolds shall not be overloaded. Barrels, boxes, kegs, and similar
unstable objects shall not be used as work platforms or to support
scaffolds.
• Overhead protection is required if employees working on scaffolds are
exposed to overhead hazards.
• The company scaffold-tagging procedure must be followed at all times.

- Rolling scaffolds
• No one is allowed to ride rolling scaffolds.
• Rolling scaffolds shall only be used on hard level surfaces.
• The height of rolling scaffolds shall not exceed four times the minimum
base dimension.
• The work platform should be planked the full width of the scaffold.
Cleat or secure the Planks to prevent their movement.
• Caster brakes must be locked when the scaffold is not in motion.
• Obtain assistance when moving rolling scaffolds. Make certain that the
route is clear. Watch for holes and overhead obstructions.
• Secure or remove all loose materials and equipment before moving a
scaffold.

- Two-Point Suspended Scaffolds (Swinging Stages)
• Employees working from a two-point suspended scaffold must wear a safety
harness and be ted oft to an independent lifeline. Multi-stage
• scaffolds require additional safety suspension lines and fall protection devices.
Ropes shall be protected from burning or welding operations.
• Suspended scaffolds shall not be less than 20 inches (50.8 centimeters) or more
than 3 feet (0.9 meter) wide.
• Wire ropes used to suspend
• scaffolds must have a safety factor of six (6) times the maximum intended load.

Electrical Safety
• All electrical equipment used on the project must be listed by an approved testing
laboratory for the specific application. All electrical equipment / installations must
conform to the National Electric Code or the equivalent country electrical code.
• Ground fault circuit interrupters (GECIs) must be utilized on all 120-volt, single-phase
15- or 20-ampere construction receptacle outlets or an assured equipment
grounding conductor program must be implemented (GFCI's must be a maximum 30
mA).
• Damaged or detective electrical tools must be tagged "Out of Service and returned
immediately to the tool room for repair. Tampering with, or the unauthorized repair
of, electrical tools or equipment is prohibited.
• Temporary lighting used in damp and/or hazardous locations shall be operated at a
maximum of 12 volts or shall be explosion-proof.
• Employees must not work on energized circuits of any voltage unless adequate
safety measures have been taken and the work operation has been reviewed and
approved by the responsible superintendent and/or the Safety & Health
Department. (See Lockout / Tag out Procedures.)

If required to work on high-voltage (over 6 0 volts) energized lines and
equipment, you must obtain prior approval from the appropriate
supervisor and the Safety & Health Department. In addition, adequate
personal protective equipment, such as eye protection
linemen's gloves, blankets, or mats shall be used as required. Temporary
lighting must have guards over the bulbs. Broken and burned-out
lamps /bulbs must be replaced immediately. Bulbs shall not extend
beyond the protective guards. Do not attempt to remove broken bulbs
until the circuit is de-energized.
• Junction boxes, circuit breaker panels, and similar equipment must be
covered and labeled
• Circuit breaker switches shall be identified as to what they control.
• Hazardous areas shall be barricaded and appropriate warning signs
posted.

Lockout / Tag out Procedures


You must follow the company's lockout / tag out procedures. The
lockout / tag out procedures require that:
• The machine or equipment be placed int h e "off" position.
• The machine or equipment be isolated from the energy source.
• A lock and tag (or equally effective control) be placed on the isolating
device.
• All stored energy (springs, hydraulic systems, elevated parts, etc.) be
controlled by blocking or bleeding down the system.
Conditions are sate to re-energize the equipment once the work on it is
complete. Never tamper with or remove someone else's lock or tag!

Environmental Protection

Employees must be aware of and comply with environmental laws, rules,


and regulations for handling materials, including hazardous substances or
wastes. Employees shall not dump, release, or otherwise discharge, or
dispose of any of such materials without the express authorization of their
supervisor and the Department of Safety and Health. Any release of
hazardous substances to the environment - whether hazardous to air, water,
or ground - must be reported immediately to the Department of Safety and
Health. Employees must take appropriate preventive measures to counter
any known environmental or health risks associated with such a release. This
may include remedial actions such as spill control, containment, and
notification to the proper authorities.

Asbestos Handling and Removal

All personnel involved with the handling, removal, demolition and / or


disposal of materials containing asbestos must be approved by the CW HSE
DEPARTMENT.
• Only authorized personnel are allowed to handle asbestos.

Confined or Enclosed Spaces

Confined spaces include, but are not limited to, caissons, storage tanks,
process vessels, bins, boilers, ducts, sewers, utility vaults, sumps, and some
trenches. What all of these spaces have in common is that they
are large enough for a person to enter, have limited means of entry and
exit, are not designed for people to work in, and can contain some form of
hazard. Some of the hazards that can be present in these spaces are lack of
oxygen, flammable or toxic gases, electrical, fall potential, and mechanical
hazards. Always make sure that a confined-space entry permit has been
completed prior to entry. This will assure that all conditions have been met
for sate entry and that an emergency Plan has been established. Confined-
space permits must be posted near the entrance and must be signed by
everyone who is authorized to enter.
You must NEVER enter a confined space without approval from your
supervisor. Only people who have completed confined-space training shall
enter a confined space. Prior to entry, a Competent Person must evaluate
the space for atmospheric and physical hazards. If hazards are present, it
may be necessary to ventilate the space and/or use air-supplied
respirators. Sources of hazardous energy must be locked out.

Never try to rescue someone in a confined space unless you have been
trained in rescue procedures and techniques. Immediately notify your
supervisor and the project safety & Health Department should an

Construction Equipment Dangers
Work performed near construction equipment (such as front-end loaders,
backhoes, trenchers, cranes, bobcats, hi-lifts, and trucks) requires special care.
Don't assume that the operator can see you. Also, because of construction
noise, don't depend on hearing a horn or
an alarm to warn you that moving equipment is near.
When you see that equipment is moving in reverse, keep out of the way and
stand clear until the operator has completed the maneuver. Never cut across
the path behind any unit while it is backing (reversing). Never ride on the
running board, steps, drawbar, or other part of any equipment, even for a
short distance. No operator should back a piece
of equipment, cranes, backhoes, front-end loaders, etc. into an area
unless/until the area is clear.
watch out for swinging counterweights on equipment. Never walk under loads
cranes and hoists. Operators are required to wear their seat belts at all times
when operating a
piece of construction equipment. Never operate any equipment in a way that
would exceed the manufacturers rated capacity.

Crane-Suspended Personnel Platforms


Crane-suspended personnel platforms shall not be used without prior approval
from the AWPT HSE DEPARTM. All other means of access must be rule out
prior to approval. Crane-suspended personnel platforms shall not be used to
transport personnel if other feasible means of access exist. The need for such
use and the appropriate safe practices shall be reviewed by the Safety &Health
Department and project management. All cranes used to hoist crane-
suspended personnel platform shall be operated and maintained in
accordance with ANSI B30.5 (current edition) and manufacturers'
specifications.

• Crane-suspended personnel platforms shall be visually inspected and
atrial lift conducted by a competent person prior to each use.
• Employees on crane-suspended personnel platforms shall wear a full-
body safety harness, which must be secured to an independent line
above the crane hook.
• An effective method of signaling the crane operator must be
established prior to use and maintained at all times when lifting the
crane- suspended personnel platform.
• Crane-suspended personnel platforms shall be rigged with a four-
point suspension capable of supporting five times the maximum
intended load.
• All employees must comply with the project's crane-suspended
personnel platform procedures.

Material Hoists
Erection and operation of material hoists shall conform to established
standards. A sign stating "No Riders Allowed shall be posted on the car
frame and at each landing. Workers are prohibited from riding material
hoists except for authorized purposes (inspection and maintenance)

Compressed Gases Use

• Always use the proper regulator for the type of gas in the cylinder.

• Attach the regulator securely before opening the valve. For oxygen, the
valve is opened fully or wide; for acetylene, the valve should be opened
only 1.5 turns. In either case, open the cylinder valves SLOWLY, and
stand to the side of the regulator.

• The threads and mating surfaces of the regulator and hose connections
should be clean before the regulator is attached.

• Always use a cylinder wrench or another tightly fitting wrench to tighten


the regulator nut and hose connections.

• Do not use a wrench to open or close a hand -wheel- type cylinder


valve. It it cannot be operated by hand, the valve should be repaired.

• Do not permit oil or grease to come in contact with cylinders or their


valves. Oily substances decompose explosively when exposed to oxygen.
Always "crack" (open slightly, then close) the valve before attaching the
regulator.

Storage

• Cylinders containing acetylene must never be stored on their side. They


contain liquid acetone that could leak out of the valves if cylinders are
not kept upright. Cylinders should be stored in a well-ventilated area
away from flames, sparks, or any source of heat or ignition.

• Keep cylinders away from electrical circuits.

• Do not store oxygen cylinders, full or empty, within 20 feet (6.2meters)


of other flammable gases unless separated by a 5-foot-high (1.5-meter-
high) barrier having a fire-resistant rating of at least 1 hour.

• Never lift cylinders by the cap. The cap is for valve protection only.

• Cylinders should be properly secured at all times (attached to a wall,


cylinder truck, cylinder rack, or post). Full and empty cylinders of all
gases should be stored separately and identified by Full and Empty
signs.

• Never store hoses in unventilated locations.

Transportation

• Cylinders should never be rolled or dragged. When moving large


cylinders, strap them to a properly designed wheeled cart to ensure
stability.

• cylinders being transported should have the regulators removed and


valve protection caps securely in place.

Before starting to burn or weld, you must inspect your work area to ensure
that sparks or molten metal will not contact flammable or combustible
materials. If you cannot provide the necessary safeguards, check with your
supervisor before staring work.
You must be sure that suitable fire protection equipment is available in your
work area.

You are responsible for maintaining your burning or welding equipment in


safe operating condition.

All welding leads and burning hoses shall be up of floors, walkways, and
stairways so they don’t become tripping hazards.

When burning or welding on coated surfaces, special precautions need to


be taken. The coating must be removed- at least 4 inches (10.2 centimeters)
- in every direction around the weld area. Contact your supervisor and/or
the Safety & Health representative for further information.

Never weld or burn on barrels, tanks, piping, vessels or other systems that
may have contained either combustible or unknown products, without first
obtaining clearance from your supervisor or the Safety & Health
Department office.
Hoses shall be disconnected from the manifold and stored properly after
each shift. Do not store in confined areas (such as an unventilated gang box)
where gasses may accumulate.

Flashback arrestors shall be installed on all cutting torches prior to use.

Welding
• Your supervisor and the Safety & Health representative must approve
the use of soft hoods for specific operations.
• Employees must wear double eye protection when exposed to flying
objects resulting from grinding or other weld-cleaning activity.
• When arc welding near other workers, all workers must be protected
from the arc rays by noncombustible screens that reduce the
transmission of ultraviolet rays.
• The frames of all welding machines shall be grounded (earthed).
• Special precautions shall be taken during Tag or Mug welding
operations to ensure that inert gases do not collect in adjacent low-
lying areas or confined spaces. Contact the Safety &Health
representative or your supervisor for special precautions.

Burning and Cutting


• Inspect equipment prior to use. If repair or replacement is necessary,
return the equipment to the tool room.
• Do not use matches or lighters to ignite torches. Spark igniters must be
used.
• Torches shall not be used to light smoking materials.
• When a wrench is required to operate the acetylene cylinder valve, the
wrench shall be kept in position on the valve while the cylinder is in
use.
• Do not use oxygen to blow off clothes or clean welds.
• Air arcing will require hearing protection. Check with your supervisor
or the Safety & Health representative.
• Turn off all valves and gauges when not in use, and disconnect hoses at
the end of each shift.

Ventilation and Protection
Welding, burning, and heating performed in confined spaces will require general
mechanical or local exhaust ventilation to reduce the concentrations of smoke and
fumes to acceptable levels. Your supervisor and the Safety & Health representative
must be consulted prior to starting these operations.
Approved respiratory protection must be worn when welding, cutting, or heating
metals having toxic significance, such as zinc, lead, cadmium, or chromium- bearing
metals.
Check with your supervisor or the Safety & Health representative.

Floor Openings, Wall Openings, and Stairways


Floor Openings
Floor openings shall be barricaded or securely covered to prevent accidental
displacement. Label or mark all floor-hole covers, "Floor Opening- Do Not Remove"
with 1-inch (2.5-centimeter) letters or larger.
All floor openings shall be covered using as minimum 3/4" (1.9cm) exterior grade
plywood, extending a minimum of 4" (10.2cm) from the edge of the entire hole being
covered. covers must be secured in place to prevent them from slipping, and hazard
warning signage to be displayed.
Floor opening covers can run continuously (trenches, pipe chases etc.... so long as the
width of the hole is not greater than 40" (101.6cm). Wherever greater than 40"
(101.6cm), the opening shall be treated as at temporary floor.
If it is necessary to work inside the barricade around a floor opening, you must use a
safety'
harness and be tied off to the structure or an independent lifeline.
Wall Openings
Wall openings from which there is a drop of more 4 feet (1.2 meters), and where the
bottom of the opening is less than 3 feet (0.9meter) from the working surface (floor),
shall be guarded with a top rail, midtrial, and toe board.
Every open-sided floor or platform 6 feet (1.8 meters) or more above the adjacent floor
or ground level must be guarded by a top rail, midtrial, and toe board or equivalent.

Stairways
Every flight of stairs having 4 or more risers shall be equipped with a top rail
and midrail on All open sides. On stairways, for every 12 feet (3.6 meters) of
vertical rise, there must be a landing platform.

Heat and Cold Stress


Heat Stress
When heat is combined with physical activity, loss of fluids, fatigue, and
other conditions, heat-related occupational illnesses and injuries may occur.
Be alert for conditions that could cause heat stress and take precautions to
prevent it. Check with your Safety & Health representative for details on
how to address extremely hot and/or humid areas. Heat stress can be
reduced by:
• Drinking plenty of cool water,
• Following a work/rest regime developed by the Safety & Health
Department incoordination with your supervisor.

Make sure you understand the signs and symptoms of heat stress. These
include:
• Heat cramps are painful muscle cramps caused by a loss of body salt
through excessive sweating.
• Heat exhaustion indicates that the body's cooling system is not working
properly. The victim will sweat heavily, their skin will be cool and moist,
and they will seem tired, confused, clumsy, irritable, or upset. The victim
may tell you that they are okay, even with obvious symptoms, because
their ability to exercise good judgment may be lost.
• Heat stroke is the deadliest of al heat stress conditions. The body
temperature will rise; the victim's skin may be hot, red, and dry; and they
may complain of headache or dizziness. They will probably be weak,
confused, and upset.

If you feel any of the above symptoms, seek proper medical attention from
the project First Aid facility.

Cold Stress
Cold stress, or "hypothermia," can occur at any time of the year. Cold stress
prevention includes:

• Dressing warmly, in layers. Protect the feet, hands, head, and face. These
parts of the body are farthest from the heart and are the hardest to keep
warm.

• Keeping dry- Feet are especially susceptible to frostbite And should be


kept dry.

• Body fatigue- If you become fatigued, your body will lose its ability to
retain heat. Be sure to replace lost fluids and calories during breaks.

• Not working alone - Use the buddy system and look for the symptoms of
cold stress in others.

• Learning what to look out for- The symptoms of cold stress may not be
apparent to the victim. The first symptoms of hypothermia are
uncontrollable shivering and the sensation of cold. The victim may also
have slurred speech, memory lapses, and drowsiness. Frostbite can occur
without accompanying hypothermia. The most vulnerable parts of the
body are the nose, cheeks, ears, fingers, and toes. Symptoms include
coldness and tingling in the affected part, followed by numbness and
change in skin color to white or grayish- yellow. Frostbite can cause
irreversible tissue damage and requires immediate medical attention.

Motor Vehicle Safety

• When you are at the controls of any company vehicle, it is important to


remember that defensive driving is a full-time responsibility.

• When bad weather affects driving conditions, you must adjust your
driving time and habits.

• Driving on wet or slippery roads is not the same as driving on dry


surfaces.

• Stay a safe distance from the vehicle in front of you.

• Anticipate driving hazards and know how to protect yourself from them.

• Be alert while driving by keeping your mind free of distractions and your
attention focused on driving; alertness involves watching and recognizing
accident- causing factors instantly.

• Motor vehicle operators and ALL passengers must fasten their seat belts
while operating company vehicles.

• Drivers of company vehicles must perform a pre-operation check (See


Safety &Health Department for details.)

• Cell phone operation including hands free is prohibited while driving a


company vehicle. Pull over to side of road (safely out of the traffic) prior
to talking on the cell phone.

• Driving under the influence of drugs or alcohol Is strictly prohibited.

• Reverse parking is mandatory wherever possible

Applying Wire Rope Clips
The only correct method of attaching U-bolt wire rope clips is shown in the
following illustration. The base of the clip bears against the live end of the
rope, while the “U” of the bolt presses against the dead end. Wire rope
must never be clipped to form a continuous loop.

• Check office furniture regularly for sharp edges, splinters, and loose
casters or bolts. The clips are usually spaced about six rope diameters
apart to give adequate holding power.

• Before ropes are placed under tension, the nuts on the clips must be
tightened to torque values recommended by the manufacturer. It is
advisable to tighten them again after the load is on the rope to take care
of any reduction in the rope's diameter caused by the weight or tension
of the load.

• The capacity of the wire rope must be reduced by the rating of the clip
used. For example, A Crosby clip is only 80 percent efficient; therefore,
the rating of the wire rope must be reduced by 2 percent. A wire rope
thimble should be used in the loop eye to prevent kinking when wire
rope clips are used.

Office Safety Practices


Keep file drawers, desk drawers, and locker doors closed when not in use.
Only open one file or desk drawer at a time .
Use handles when closing files, desk drawers, and sate or vault doors.
Watch for telephone cords, power cords, office machine cords, and
computer
cords or cables, waste baskets and other hazards which may cause tripping.
Be careful of swivel chairs. Don't slump back in them without testing your
weight gradually. Walk (don't run) in corridors or on stairs, and use handrails
where provided.
