May Watts Elementary School eReader Permission Form 2012-2013 School Year

My student, ____________________________________(name) __________________(class) has my permission to bring his/her eReader ______________________(brand) to school.

We confirm that the eReader is permanently labeled with student identification. We agree that the device will be used exclusively as an eReader. We agree that the device will not be connected to the districts wireless network. We agree that, if the device has 3G or 4G Internet capability, the student will not use it to connect to the Internet. We understand that the sharing of eReaders is discouraged. My student takes full responsibility for his/her personal property, which is brought to school at the owners risk. The school and district are not liable for damage to or theft of the device. We understand that the device will only be used in designated areas for instruction related activities, approved of and supervised by the teacher in charge. We understand that the device will not be permitted in the lunchroom or at recess.

Failure to abide by these policies will result in loss of eReader privileges for the remainder of the school year.
