Immaculate Conception, Cubao, Quezon City, Philippines Dear Madam: As part of our requirements for our clinical psychology class, we are requiring our students to conduct a psychological evaluation of one (1) institutionalized patient. The main purpose of this activity is to give our students an experience of how to conduct an evaluation in an in-patient setting. May I humbly request your good office to indulge us in our need by providing our students a patient to whom they can administer their psychological tests, with which upon they can base their evaluation. All of them have been already provided with the psychological tests that they would administer. Also, may we request that you provide patients that are above 18 years of age, so that we would not have any problems with the consent form that we have provided. We thank you for your understanding and kind consideration that can be extended to our students, and we appreciate your help in honing the skills of our future psychologists.
