Scenery/Properties Check Out Form

I, ______________________________ am responsible for the safety and security of all scenery and/or properties checked out to me. I fully acknowledge that I am wholly responsible to return all materials and items checked out by me in the same condition as received by the due date. I will not engage in any illegal activities with checked out scenery/properties and I will not put materials/items at risk, damage any property or leave unsecured. Any damage done to, or loss of the scenery and/or properties can result in loss of check out privileges and financial compensation to ASU as pertaining to repair or replacement of materials/items. If scenery/properties are damaged and/or lost, a bill will be sent to student for payment. Failure to pay will result in the withholding of transcripts and diploma until payment arrangements have been made. By signing, you have agreed that all scenery and/or properties checked out are for the purpose of completing a class assignment or project, assigned by a faculty or staff member for a production class only. Materials and items are not allowed to be used for personal projects, unpaid or otherwise.

Arizona State University or The School of Theatre and Film is not responsible for any negligence on the part of the individual checking out any scenery and/or properties.
