Fatal 5 Accident Special Safety Message Applicable to all business units.

. On 02-07-13 an incident occurred at Enlow that resulted in severe injuries to an employee. The incident occurred while an employee was positioned in a place not acceptable as a work location during equipment operation. The individual was positioned on the top of a continuous miner between the bolter station and the conveyor examining for a hydraulic oil leak while power was on the equipment and the pumps in operation. The investigation is ongoing but preliminary information revealed that while the individual was positioned on the miner, the conveyor was started. The injured was carried by the conveyor into the spray bar and partially into the conveyor tunnel prior to him being caught and held in position while the conveyor continued to run until coworkers was able to stop the conveyor using the emergency stops. The individual received crushing type injuries and has undergone some surgery. Please remember this individual and his family in your thoughts and prayers.

Going forward beginning on the afternoon shift conduct a safety meeting with all personnel stress the importance of identifying red zones in their individual work locations and the requirement to take necessary actions to eliminate the hazards and the red zone prior to accessing the area. If you must access a nontraditional area of any equipment, such as on top of the machine, the equipment must be stopped and proper steps to de-energize the equipment, lock and tag and try out. This safety requirement is applicable to all Business units.
