Incident Description
On the morning of 14 February 2013 in Area 4-3 along the North West, outer slope of the bund wall of
Tank F-053, Ready Mix Company was engaged in concrete pouring activities to complete the unfilled panels
of the said bund wall. 2001 model Scania CIFA K3-XL/36 concrete pump truck was pumping the concrete from
a Ready Mix concrete mixer into the unfilled panels of the bund wall. Two (2) workers were to be engaged in
concreting and finishing/smoothing the bund wall with the presence and close supervision of foreman and
site supervisor. The work group was positioned under and around the boom during the time. At 09:27,
the first section of the boom failed and tore, collapsing onto three (3) employees below. Two (2) employees
sustained fatal injuries. The third IP was sent to hospital and released the same day.
Recommendations
All equipment must be thoroughly inspected before operation, it must be certified by approved third party
consultants.
The importance of education and training must not be underestimated. With the relevant information and training,
employees are made aware of what to look out for and avoid, especially line of fire. These are hazards that need to
be actively identified and avoided. This will help minimize recurrence of serious incidents in the future.
Employees need to be spatially aware of their surroundings and ongoing activities at all times. They need to
actively avoid working under suspended booms.
Adequate communications must be ensured at all times between all parties involved in a task. This includes
verbal (toolbox talks and stand downs) and written, in the form of method statements and TRA’s. All persons
involved in the undertaking of an activity need to sign the daily toolbox talks and TRA’s, conducted in the appropriate
languages, to show that they have attended, are aware of and understand all hazards and risks involved with a task as
well as the steps to take to minimize them. All written documentation needs to be updated regularly and approved to
ensure newly found risks are identified and mitigated in a timely manner.
