Description Every year, it is evident that numerous incidents relating to tipper truck overturning occur as a result of mechanical failure to the hydraulic jack as well as due to unsafe tipping operations, which has a high potential to cause serious OSH incident and other consequences. The common cause of these type of incidents, includes (but not limited to), Mechanical failures such as breaking of telescopic jack/lock during tipping operation, Equipment positioned on uneven and unstable ground, Inadequate servicing and maintenance of the equipment such as poor tire condition, hydraulic leakage, etc., Unsafe driving such as over speeding or sharp turning, Sudden and rapid tipping of the load by the driver, driving with elevated/raised bucket, etc. Control measures to be implemented to prevent incident includes (but not limited to): Specific Method statement, risk assessment, Drivers’Fatigue Prevention management and safe working procedures shall be developed, approved and implemented. Ensure the availability of equipment registration and mandatory competency certifications (operator & vehicles/equipment), written task-specific safe working procedures, Traffic management plan and Training records for all employees involved in tipping operations. Implement a robust pre-use and regular inspection regime including periodic servicing/maintenance for all the vehicles and equipment entering the project and use any identification method to ensure effective implementation e.g., inspection tag. Competent supervision and sufficient resources shall be deployed to set up and monitor/control an appropriate exclusion zone around tipper trucks and equipment during tipping operations and avoid trucks tipping close to each other. All tipping activities shall be guided by a competent Banksman as well as under the direct supervision of competent personnel (Supervisor/Foreman) and they should always position themselves in a safe area away from trucks (Dangerous or blind zone). Avoid delivering materials after working hours if there is no competent supervision and banksman to guide the operators. Ensure that tipping is carried out on a firm level ground - Uneven/unstable ground is a common contributor to overturning. Provide training to every personnel that are engaged with the tipping activity to alert them of the hazards and risk associated with activities, this includes the safety precautionary measures that must be adhered. Medical Surveillance shall be undertaken for all driver as per OSHAD-SF CoP 5.0 requirements. Adverse weather conditions such as wind, etc. shall be considered prior tipping operation, since heavy winds might affect the stability of the truck. Emergency procedures should be in place and reviewed regularly to ensure they are appropriate along with a recovery plan. For Drivers / Operators: Possessing UAE valid driving license. Moreover, before dumping/tipping, operators should ensure that the tailgate is unlocked and that the vehicle is on a reasonably leveled surface to avoid tip-overs, Always observe travel areas to ensure no personnel is walking or standing behind the equipment, Keep mirrors and windows clean to ensure adequate visibility, Ensure all safety features are functioning appropriately E.g. reversing alarm and Listen for backup alarms from other equipment, Report defective alarms, Check for any overhead electrical power lines and avoid tipping in such locations, Make daily and spot walk-around examination before getting into equipment and starting up, Observe all stipulated speed limits, If needed lower speed to adjust to the weather and road conditions, Always wear seatbelt and required PPE’s and Ensure that daily vehicle/equipment checks are completed, Follow the direction and instructions given by banksman, driver should not leave the truck unattended while tipping, Drivers and operators should report all incident along with unsafe acts and conditions etc. For further information please refer to OSHAD-SF (Code of Practice) CoP 25.0 Driver Fatigue Prevention, CoP 36.0 - Plant and Equipment, CoP 51.0 - Powered Lift Trucks, CoP 44.0 Traffic Management and Logistics, CoP 33.1 Traffic Incident Site Management Issue Details: The information in this document is to be distributed to all relevant key personnel within your organisation, in order to be read / understood, then communicate to all personnel under their control and supervision for understanding and implementation.
