A. Analysis of Feasibility (Complete ALL the following questions)
Who created the script? (Name and Company) Is this Script generic or specific? Is there a manual way to do it? If there is a manual way to do it. What is the effort? Which Legal Entities, Operating Units, Organizations, GL Books or FA Books will be affected?
Is this script skipping or ignoring any business rule? (ex: WF Approval, Max or Min Validations)
Are you using IDs or Codes? If you are using IDs, Did you validate the IDs in Production?
Which APPS User are you using to put in the LAST_UPDATED_BY? (Must be the actual business user)
Which Date are you using to put in the LAST_UPDATE_DATE?
B. Technical Impact (Include all the tables affected by the script)

E. Comparison Analysis with Changes in the Application If there is a manual way to do it, you have to include: a. Attach the TRACE when you apply the changes from the application
b. Attach the Comparison analysis of the tables before and after the changes
F. Instructions a. If the solution is recommended by Oracle. The SR has to be attached to the CO. b. If not. Code Migration Form is required and the script has to be in $DMF_%_TOP/Install
