PCL SAFETY INSPECTION

Flint River; Albany, GA Walk Through with Larry Fortier, TIG Tampa, District HSE Manager May 27, 2014

GENERAL NOTES FROM WALK THROUGH
Minimize Handling
Material/debris should not pass construction entrance/property
Repair and organize materials so they can be used and accessed quickly
Exposed nails need to be addressed immediately to avoid stepping on or brushing against
Since there are many different types how are the trash transportation bags on site to be moved and what
are they allowed to contain?
If a slope can be made instead of handrails, this is preferred because it requires less maintenance and
man hours
Because snakes have been sited, a safety review should be done about snakes and reptiles. A short
explanation and plan should be introduced so workers know what to do.
Toe boards are used to keep small material from falling over an edge. Additional height may be required in
an area experiencing heavy waste.
Excess materials and debris must be maintained and organize especially close to the entrance
Duct tape should always make one curious
Use the tying method when connecting extension cords together rather than duct tape
Field made railings should be able to support at least 200lbs of outward and downward pressure for
handrails. Also, the post should be no farther than 8ft O.C. apart.
Pay attention to construction and egress paths. Ask: Where is material going? Is it staged in a logical
manner? Can workers safely maneuver around the site?
GORY = Green: Jan-March, Orange: April-May, Red: July-Sept, Yellow: Oct-Dec. This is the inspection code
PCL uses to make sure equipment and tools are up to date on inspection. Any old colors should be
removed so inspection period is clear.
Always but especially when working in a tight/small area keep the work area clear and organized, ask:
What are we not going to use today?
Hand injuries are the most common construction injury. To avoid this follow: Do not put your fingers
anywhere your eyes havent been.
Have positive control of the task at hand. Instead of just saying be care, come up with a strategy or
physical way to make things better.
Real estate is at a premium; everything must be organized.
Covers over holes must be strong enough to withstand twice the weight of the anticipated load. This can be
achieved with not only a flat cover but, when appropriate, several wood members as well.
Ladders must extend a minimum of 3 feet (3 rungs) beyond the opening to give the climber something to
hold onto when exiting and should be tied off/secure on both top and bottom.
Keep up maintenance on all equipment and tools. Repair broken items immediately.
A confined space is defined as an area which is enclosed with limited access making it dangerous. Since it
is not logical that the open end of the bridge span is an egress point, the diaphragm is an enclosed space
and requires licensing and special regulations.
Make is sound important, it will be important.
If shut off valves on hoses do not work properly, the only way to shut off substance would be at source.
This delay may cause a disaster that could easily be avoided.
When egress exists by climbing over items (beams) over 19 it is a good idea to make easier access over
these areas.
Install appropriate rebar caps whenever penetrating size rebar is present.
Use appropriate connecting devices. Just because a creative solution is found does not mean it is the
safest one.
Roll up all unused extension cords and hoses. These are a major trip hazard.
If you have time to lean, you have time to clean.
Dont expect it, demand it.
Plan, Analyze, Execute, Reanalyze
It has been proven that when safety and organization increases, quality and productivity increases
Implement daily, even hourly, controls to monitor housekeeping and safety controls
