Interview / Oral Assessment (any 4 questions)

1. If you are working in a project that involves handling and storing Compressed Gas

Cylinders (Oxygen, Acetylene, Argon, etc.), explain how you can do that in a safe

manner. (10 points)

Compressed gas cylinder shall be kept securely stacked inside the purpose built rack in upright

position to prevent accidental fall. They must be kept away from source of heat or open flame. A

protective cap should always be kept in place while transporting the cylinders. They should be

transported through suitable trolley. They should be segregated between filled and empty

cylinders. They should be segregated between oxidations agents (Oxygen, nitrous, etc.) and

flammables (Acetylene, hydrogen, butane, LPG, etc.)

2. If you are supervising an operation that includes “Crane and heavy lifting works”,

describe any cases or conditions which shall force you to suspend the work so

that the safety of the workers is not compromised. (10 points)

Crane and heavy lifting works shall be suspended under the following conditions:

• Raining.

• Dusty.

• Foggy.

• High wind.

• Dark/ night.

• Hi-Temperature/ Hi- humidity.


3. Describe how you would act in this scenario:

While working on site one the workers has found an unidentified object and brought

it to your attention. You suspected it to be an unexploded ordinance, list

precautionary measures you would take to minimize the dangers associated with

discovery of suspected Unexploded Ordnance, EOD. (10 points)

Hazard of EOD is possible explosion which may lead someone to die or face serious injury.

When someone finds suspected EOD, he is supposed to:

• Stop working

• Not touch suspected EOD

• Install barricade system

• Inform Area fire station

4. Explain the precautionary measures you would follow at site when dealing with

“Excavation works” to ensure the safety of your workers. (10 points)

We need to have “Excavation notification” regardless the depth of the excavation. Adequate

fencing, barriers and warning notices are to be provided around any excavation. If the depth of

the excavation exceeds 1 meter, shoring must be used. Excavated material shall be placed at a

distance not less than 1 meter from the edge of the excavation trench. All equipment must be

kept clear from excavation to prevent accidental fall. Any equipment shall be kept clear from the

excavated area by at least 3 meters away. Excavations must be inspected on a daily basis.

For any excavation deeper than 1 meter, suitable ladder must be positioned projecting a

minimum of 1 meter above the edge. Ladders shall be provided every 25 feet of lateral travel in

the trench.
5. Explain the necessity of Tool Box Talk related to efforts for minimizing incident in

working place. Frequency? Level of attendance? And timing of Tool Box Talk

given. (10 points)

Tool Box Talk is a reminder and a training media given by Supervisor/ Foreman to his

subordinates/ workers prior to start working. It usually takes 10- 15 minutes duration in the

morning. Generally, Tool Box Talk is to explain hazard and risk associated with job to be done

during that day, so, that incident can be minimized. Tool Box Talk is given on a daily

basis and inviting all levels of workers (Carpenters, civil technicians, labors, etc.) who work in

that small working group.

6. Describe the most difficult situation you faced in your career. How did you handle
it.?

Answer should reflect applicant’s logical thinking and safety oriented behavior.

7. If the work carried on site involves the use of a crane, which part of the crane is
used to stabilize it before the lift and what should be its position. When you
inspect the lifting tackles, what should be the most important information tagged
on it?

Answer (first part of question): Outriggers, They should be fully extended.

Answer (second part of question): SWL (Safe Work Load).
