Procedures for Late Reporting of Grades (for the students):

1. Fill-up the completion form of PUP (can be downloaded from the PUP website).
2. At the upper right corner of the completion form, check the Late Reporting of Grade box.
3. Have the class record and the former grade sheet of the subject with the deficiency. (It
will serve as your proof of having your grade)
4. Make sure that the class record and the completion form have the validation of the
professor. The class record must have the signature of the professor.
5. After filling up the completion form, provide 4 copies of it.
6. Then go to the cashier and present your ID and your completion form and pay P30.00
7. After paying, you will have a receipt, provide 4 copies of it.
8. Submit the completion forms and the receipts to your departments office. The
department will submit it to the dean.
9. Wait for the result in the SIS. If you still dont have the grade, go to your department to
check the status of your grade.


General Policy
The student shall have his/her grade in the class record of the professor. Otherwise the
student cant have his/her grade.
