Job Description

Support Worker
Work Area: Designated Locality/Neighbourhood
Hours of Work: As agreed
MAIN OBJECTIVES OF THE POST
1. To provide high quality personcentred support to individuals! both "ithin
their ho#e and in the co##unity! in accordance "ith the $%&'s ai#s and
principles and the (odes of (onduct and )ractice issued by the *cottish
*ocial *ervices (ouncil+
2. To help individuals plan their support! accessing both for#al and infor#al
support net"orks! and to pro#ote the rights and choices of the people you
support+
3. To constantly #onitor the quality and appropriateness of support! and to
ensure good ,tea#"ork' is developed and #aintained bet"een all people
involved in a person's support+
RESPONSIBIITIES ! "it# peop$e "#o %re supporte&
1. To build "ar#! caring! and positive relationships "ith people receiving
support! and to keep the individual's best interests in #ind at all ti#es+
2. To provide direct practical help! advice and guidance to individuals in all
areas of their life as required+ This support should at all ti#es respect the
person's rights and personal dignity and value! and should be provided in
a #anner that is entirely nondiscri#inatory! sensitive -to needs and
"ants.! i#age enhancing! and e#po"ering+
3. Areas of support activity #ay include/ personal care -including inti#ate
areas of selfcare such as "ashing! dressing or using the toilet.! do#estic
ho#e care! 0nancial #atters -including #a1i#ising inco#e and #anaging
e1penditure.! e#otional and physical "ellbeing! education or e#ploy#ent!
and general social or leisure areas -including #aking and #aintaining
friendships/relationships.+
4. To "ork creatively to help individuals recognise achieve#ents! celebrate
successes! assist infor#ed decision #aking! identify proble#s and 0nd
"ays of resolving the#! and to take advantage of opportunities! gain ne"
e1periences! and develop lifeskills and con0dence+
5. To use personcentred planning tools to help individuals -along "ith other
i#portant people in their lives. identify future aspirations! and current
support needs and preferences! and consider ho" these can best be #et+
6. To support individuals to coordinate their o"n for#al and infor#al support
-fa#ilies! friends and other interested people.! or "here necessary or
desired to coordinate this support on their behalf+
7. To liaise "ith other agencies "ith and on behalf of individuals to enable
access to appropriate resources and services -including specialist
services.+
8. Where individuals share acco##odation to help the# live together
positively! share responsibilities -including running their ho#e.! identify
co##on issues and concerns and 0nd solutions to proble#s+
9. To support advocacy -"hether it be self advocacy! citi2en or group
advocacy.! and to pro#ote active participation "ithin $%&+

1. To co##unicate e3ectively! and to "ork positively and cooperatively "ith
#anagers! other e#ployees! "orkers fro# other agencies and #e#bers of
the public -including fa#ily and friends of individuals "ho are supported. 4
and to keep supported individuals as the #ain focus of your e3orts+
2. To take your personal and professional develop#ent seriously! at "ork this
"ill include your active involve#ent in supervision! and also -"here
identi0ed as appropriate. taking part in #eetings! foru#s! and training+
3. To #aintain records in a "ide range of areas -including using non"riting
based #ethods such as graphics/photos! audio/visual tapes! co#puters
etc5.+
4. To understand issues of con0dentiality in relation to your "ork and
individuals "ho are supported and to ensure this is #aintained+
5. To provide support! or be available to provide support! in line "ith agreed
practice/local arrange#ents/ "hich is likely to include participating in a
rota! sleepover cover! and a range of 01ed ti#e or 6e1ible ti#e
arrange#ents! including oncall arrange#ents+
6. To help ensure that people's ho#es are safe! secure and co#fortable by
o3ering support! undertaking tasks directly! and "here appropriate
arranging servicing/repairs! and/or advising line #anager of concerns or
reporting direct to relevant others+
7. To be a"are of the Health and *afety )olicy of $%& and understand your
o"n responsibilities "ithin this! and to actively use risk assess#ent
practices -to avoid or #ini#ise risk. in all areas of your "ork and support+
8. To be a"are of and follo" all procedures and practices of $%& -as laid out
in the )rocedures Handbook.+
9. To conduct yourself in a #anner consistent "ith the e1pectations of other
bodies -eg! *ervice Level Agree#ents/(ontracts "ith funding agencies! the
(are 7nspectorate! and *cottish *ocial *ervices (ouncil.+
10. To undertake other duties within the main objectives o the !ost as re"uested b# !eo!$e #ou
