Before a case can refer to the CGT and
proceed in a counseling session,

Questionnaire
By using an inventory to study a situation
where the present issue taking place

Normally, Section A will be personal details
and for Section B the related and relevant
inventory is implement.

Objective- justify the respondent-prepare
the questionnaire/instrument-implement-
collect and analyze-report and suggestion-
action plan
