Scenario
A

Boy, come to primary health at date December 5 2009,


because of fever 1 day. The child birth at February 25,
supported by midwife, gasp for breath, with weak tonus, birth
weight 3000 grams, birth length 49 cm, head circumference
35 cm, Body Weight 6500 grams, Body Length 70 cm, head
circumference 42 cm, body weight 2 months ago continued
6100 grams, 6300 grams was taken. For daily meal the baby
was fed with rice and vegetables, tofu, tempe, and sometimes
egg. Starting from the age of 3 month, he consumed formula
milk, bananas, and baby porridge. Imunize BCG at 2 months,
polio 4 X, hepatitis B at 40 days, 2 months, DPT 3 X. The child
can roll over, can not sit and stand holding, turn to voice and
turn to rattling sound, cannot feed self biscuits and peak a

Scenario
He got BCG immunization when he was 2 months old; 5 times
polio vaccine, the last time was during PIN period; B Hepatitis
vaccine twice, in the age of 40 days and 3 month; DPT when
he was 4 and 6 months old.
The baby was able to crawl, but not yet sit and stand by him
self. Sometimes mumbled, not able to hold jingling toys with
his hands. Can not feed him self with biscuits, and do not
know how to play peek-a-boo.
His rooms windows were always closed to avoid the baby got
sick, the ventilation was perfectly cover with papers to avoid
mosquitoes.
Toys available at his house: jingling toys, dolls, a three wheel
bicycle. The mother never talked much.
