During some routine maintenance of our files, we noticed that the address we currently
have for your child lies outside of North Stars boundaries. The address we have is:

When a childs address is out of our schools boundaries, it becomes necessary for a
parent/guardian of that child to fill out a special permit so he/she can attend school
here. We have gone through our folders, and it appears as though we do not have a
permit for your child. Please review the following scenarios to see which accurately
describes your situation:
1. Have you moved to a different address that is within the boundaries of the
school? In this case, a special permit is unnecessary. Please come to the office
with something that shows this new address (a bill, for example) to update this
information in person.
2. You do live outside of the boundaries, and you have filled out a special permit. In
this case, we may have misplaced the form in our files. Please come to the
office so we can make a copy of the form you filled out for our records. You can
also fax to us at (801) 578-8445. We apologize for the inconvenience.
3. You do live outside of the boundaries, and you have not filled out a special
permit. In this case, please come to the office to fill out a special permit. There
is a $5 fee to process this form unless your child is on free/reduced lunch. If that
is the case, there is no fee.
This needs to be accomplished before February 15th. If you have any questions or

Al hacer mantenimiento de rutina de nuestros registros, nos dimos cuenta de que la

direccin que tenemos asignada a su hijo est situada fuera de los lmites de North Star.
La direccin que tenemos es: ___________

Es necesario que un padre/tutor legal del nio llene una autorizacin especial para que
el nio pueda asistir aqu cuando la direccin est fuera de los lmites de nuestra
escuela. Hemos revisado nuestras carpetas, y aparece que nos falta una autorizacin
por su nio. Por favor revise los escenarios siguientes para ver cul describe
correctamente su situacin:
1. Se han mudado a una direccin diferente que est situada entre los lmites de
la escuela? En este caso, la autorizacin no es necesario. Necesitamos que un
padre/tutor legal venga a la oficina con algo que demuestre esta direccin nueva
(por ejemplo: una factura) para actualizar esta informacin en persona.
2. Viven fuera de los lmites, y ya han llenado la autorizacin especial. En este
caso, quizs hemos extraviado la forma en nuestros registros. Necesitamos que
un padre/tutor legal venga a la oficina para que podamos hacer una copia de la
forma que l o ella llen. La necesitamos por nuestros registros. Nos la puede
mandar por fax, tambin a (801) 578-8445. Pedimos disculpas por la
inconveniencia.
3. Viven fuera de los lmites, y nadie ha llenado la autorizacin especial. En este
caso, necesitamos que un padre/tutor legal venga a la oficina para llenarla. Hay
un pago de procesamiento de $5 a menos que su nio reciba los almuerzos
gratis o de precio reducido. Si tal es el caso, no hay un pago.
