Subject: Reason for condonation of delay for approval of disability leave

Respected Sir,
This is in reference to approval of disability leave for the period of 29.11.2011 to
23.12.2011 and the condonation of delay for the same. In this regards, I would humbly
like to make the following submission:
1. During the tenure at Rajkot area office as sales officer, undersigned along with Mr. S.
Khandare, AM (P) as RTD committee were assigned to carry out RTD from Sanand
Bottling plant to Sayla market at Surendranagar district on 28th November 2011.While
carrying out RTD , we met with an accident when our car was hit by a truck at Sayla.
Consequently, undersigned was admitted to Sterling Hospital, Ahmadabad from 28th
November, 2011 to 30th November 2011.
2. As I resumed duties on 26.12.2011, because of trauma and mental disturbance recovering
from the accident, my office provided me full support and cooperation during the period
and the incident and related issues were not discussed then at the office .I also understood
then that this being an on duty accident, the period of absence would be regularized by
the office and thus was unaware of specific need to apply for such a leave then.
3. Later in April 2012, four months post my resuming of duties , I was transferred to Baroda
Sales Area ,Surat Area Office and then there was no intimation of such regularization and
the leaves were not also deducted then. Only in Mar 2013, I came to know that the leave
records for period of absence after accident i.e. from 29.11.2011 to 23.12.2011 has to be
updated with a deduction of 25 days. Thus I immediately initiated a note for approval of
disability leave on March 2013, while the leave records got updated in June 2013 with a
deduction of EL for 25 days.
Sir, I would also kindly like to bring to your notice that due to the accident with my
colleague expiring in the accident, I have been in mental trauma apart from physical
medical issues for quite a long time , that has also affected my health and body.
Deduction of these leaves for the period of absence after the accident as EL will add to
the loss that I have faced.
In view of the above mentioned genuine reasons and with earnest humble request, I
request you to kindly condone the delay and approve the application for disability leave
and kindly oblige .
