I hereby certify that I have rendered the following duties for the period of
OCTOBER 1-31, 2014:
1. Orientation to the NSTP: R.A. 9163 and the Implementing rules and
regulation (IRR) of the NSTP.
2. Provided activities that are designed to highlight values and skills
formation, especially of the teamwork and leadership.
3. Motivate the students to represent in activities sponsored by the
school and the city government.
4. Prepared the 2014-2015 work together as potent resource group in
the community service.
5. Supervised student activities outside the university and the
community service.
6. Performed any tasks assigned.
