Goal Making for Personal Health Lesson Plan

Description: Each student will make a goal related to dental hygiene and over the following week, fill
out a Goal calendar with their parent/guardian.
Subject: Health
Grade: 2nd grade
Duration: 30-45 minutes and 5-10 minutes a week over a month
Indiana Academic Standards: 2.6.1- Identify a short-term personal health goal and take action
toward achieving the goal.
Objectives:

The Student will be able recognize and recall a short-term goal for personal
health and take action to complete the set goal.

Prior Knowledge:

The student will need to have prior knowledge of writing skills, simple sentence
making skills, and how to read a calendar

Materials:

The student will need a Goal Making sheet, Goal Calendar, and a pencil to
complete this assignment.

Procedure:
Opening Set/Anticipatory SetThe lesson will begin with me asking the students if they have ever done a personal goal
and if so have them explain their personal goal to the class/if not then I will explain one that I
did as a child (to save up money to buy my guinea pig). Then tell them that we are going to
do short-term goal over the course of a month over dental hygiene (example- brush your
teeth twice a day of floss once a day).
Instructional Sequence
Start by handing out the worksheet (ask one of the students to do this). Then talk about
goals and how they will not only help in school but outside of it as well. Explain the directions
for this activity- (They will each set a goal that has to do with dental hygiene. After confirming
that their goal is appropriate with me, they will get a Goal Calendar that they will have their
parent/guardian to sign to say that they have done the said goal.) Then once a week, the
entire class will see where their classmates are in their goal.
Independent Practice
Each student will need to do their Goal Calendar with the help of their parent/guardian
outside of class.
Closure
Following the closure of the month, each student will bring in their completed calendar and
the class will as a whole go over their goals and each student explain why or why not they
complete their goal. To reiterate the objective of this lesson, any student who wishes to get
extra credit will have the opportunity to make personal goals and do a Goal Calendar to
completion.
