CAP
The Community Alternatives Program for Disabled
Adults is a program available through Medicaid
under the Community Care Section of the Division of
Medical assistance.
For adults who are at risk for institutionalization
CAP provides an alternative for this target
population to stay in their primary residence.
Brings resources of Southeastern Health into a
patients home

HISTORY
Program began in 1982 in only four counties
Now available in every country in North Carolina

QUALIFICATIONS
In order to be accepted as a qualified participant for CAP you must
Be receiving Medicaid
Pass assessment screening
Choose CAP over institutionalized care
Documentation related to medical diagnosis and identification of

medical needs
Can have his or her health, safety, and well being maintained at

home within the Medicaid cost limit
