Mechanism of
Action
Diltiazem
produces its
antihypertensive
effect primarily
by relaxation of
vascular smooth
muscle and the
resultant
decrease in
peripheral
vascular
resistance. The
magnitude of
blood pressure
reduction is
related to the
degree of
hypertension:
thus hypertensive
individuals
experience an
antihypertensive
effect, whereas
there is only a
modest fall in
blood pressure in
normotensives.
Adverse Effects
Indication
Dizziness,
lightheadedness,
weakness, nausea,
flushing,
constipation, and
headache may
occur.
The patient is
hypertensive. Her
regular Bp is
around 140/100
so Diltiazem is
given orally to
reduce her blood
pressure.
Nursing
Responsibilities
>Check BP and
ECG before
initiation of
therapy and
monitor
particularly
during dosage
adjustment
period.
>Lab tests: Do
baseline and
periodic liver and
renal function
tests.
>Monitor for and
report S&S of
CHF.
>Monitor for
headache. An
analgesic may be
required.
>Supervise
ambulation as
indicated.
