I, Mr. Khan Muhammad S/O Sher Khan resident of village & P.O.
Multan Khurd Tehsil Talagang District Chakwal , presently
serving as Headmaster at Govt. Boys Elementary School
Khuian Tehsil Talagang District Chakwal do near by solemnly
affirm and declare on oath as under .
1. That I am proceeding to avail ex-Pakistan Leave for the
Hajj .
2. That while at stay aboard I shall not join any service abroad
.
3. That I will stay not demand any pay etc. in foreign currency .
4. That I will come back.
