FIELD TRIP PARENTAL / GUARDIAN CONSENT FORM AND LIABILITY WAIVER

I am allowing my child
to join/participate in the
field trip to the Bataan Nuclear Power Plant in Morong, Bataan on November 7, 2015 under the
supervision of Mr. John Ephraim Torres, a faculty member of UP Manila College of Arts and Sciences.
I waive any claims for damages which my child will have and will hold the university no
responsibility from any and all liability, claim, suit or action from any unforeseen incident any event
beyond their control, not attributable to negligence or wilful and deliberate act of the latter, or that
arising from the students disregard or non-compliance with rules, regulations and guidelines on
field trips, as well as on the precautionary measure(s) taken by the Faculty and Coordinator-InCharge.
