HSE Statistic Definitions


Lost Time Incidents

Injuries where employees at Dunfermline (including core contractors) are off work for more than
3 days.

RIDDOR Recordable

UK requirement under The Reporting of Injuries, Diseases and Dangerous Occurrences


Regulations 1995 where we have a duty to report work-related deaths, major injuries or overthree-day injuries, work related diseases, and dangerous occurrences (near miss accidents).

Corporate Reportable

Injuries that fall under the U.S. Occupational Safety & Health Administration (OSHA) regulatory
bodys criteria of lost time injuries over 1 day. OSHA is the U.S equivalent to the UKs Health
and Safety Executive (HSE).

Major Injuries Minor Injuries -

Injuries that include fractures (other than to fingers, thumbs or toes); amputation; electrical
shock/burn related injuries; injuries requiring admission to hospital for over 24 hours; certain
acute illnesses and diseases.
First aid cases.

SEPA Recordable Environmental Incidents -

Incidents where if we cannot contain or clean up our own environmental incidents, the Scottish
Environmental Protection Agency (SEPA) needs to be informed. For example, contamination of
surrounding watercourses, land or air.

Dangerous Occurrences Unwanted Condition Reports


Exposure Hours

Incidents that do not result in injury, but clearly could have done. They include - electrical short
circuit or overload causing fire or explosion; failure of radiography or irradiation equipment to deenergise or return to its safe position after the intended exposure period; malfunction of
breathing apparatus while in use or during testing immediately before use; explosion or fire
causing suspension of normal work for over 24 hours
Safety alerts and accident/incident report forms (1009 forms).
This is a calculation of the total hours worked including weekly staff, monthly staff, contractors
and total overtime hours worked.
