Student Strengths/Interests/Likes:

Ann is energetic and enthusiastic about learning, she loves Frozen (the movie), math
time, and snack!
Ann is a first grade student with excelled math skills, at the beginning of the school year
she could do 30 flash cards with 28/30 accuracy, she is currently able to do 50 flash cards
with 49/50 accuracy.
Ann can only recognize 3 out of 15 sight words.
Ann seems to find it very difficult to keep her hands to herself during group meeting.

Accommodations/Modifications:
(Include state testing levels)
Extra practice on sight words during the first 5 min. of recess. One half hour of resource
room instruction 3 times a week.

Resource Room Schedule:


Monday- Friday 9:30-10:00.
Paraeducator Support in General Education Classroom
(Include times as well as responsibilities of the paraeducator as they relate to this
student.)
Para will assist student with staying on task, when required, and reading practice before
recess.

Other: (special procedures, techniques, equipment, etc.)


Sight word flash cards, books, and letter activities are sent home with Ann to work on
before bed and on the weekends.
