TO WHOM IT MAY CONCERN:
This is to CERTIFY that there was price escalation and/or underestimated Program of
Work (POW) resulting to the non-completion of the Sub-Project, Improvement of Barangay
Camambugan Farm to Market Road of Barangay Camambugan, Ubay, Bohol.
This is further certifies that the cost of contingency cost needed is Fifty seven thousand
nine hundred five and 50/100 pesos (P 57,905.50) and that said amount was not part of the
committed local counterpart contribution of the community. This cost will be used for the
installation of additional RCPC 610mm diameter to provide enough drainage system for the said
access road.
This certification is issued to support the request of the community for the release of the
contingency cost in compliance to KC FM Guidelines No. 16 series of 2007 which is likewise
part of the documentary requirements for the Request for Fund Release (RFR).
