1 Does the facility own or operate a public drinking water system? 2 Does the facility own or operate a public drinking water well system? 3 Is the facility required to perform drinking water monitoring? 4 Does the facility respond to water quality complaints? 5 When responding to water quality complaints, does the facility perform drinking water sampling and analysis? 6 Are all drinking water samples analyzed by a certified laboratory? 7 Are analysis results reported to complainants? 8 Are analysis results maintained onsite for 10 years? Backflow Prevention Devices 9 Are backflow prevention devices installed to protect the public water system when irrigation systems, drinking water systems, fire protection systems, and other water systems that may contain possible contaminants are connected to public water? 10 When installed are backflow prevention devices tested, prior to water service being activated, by an SC DHEC certified tester and results submitted to the Columbia Cross Connection Control Program within 10 days after installation? 11 Are all backflow prevention devices tested by an SC DHEC certified tester at least one time per year? 12 Is a copy of the annual test results for each backflow prevention device forwarded to Columbia Cross Connection Control Program, as required?
