Outstanding Ombudsmen/Key Volunteers

RECOGNITION CRITERIA INFORMATION


The San Diego USO, San Diego Armed Services YMCA, Navy League Council of San Diego, and
the Fleet & Family Support Center will host a Luncheon and Recognition Ceremony for
Outstanding Ombudsmen/Key Volunteers at the Paradise Point Resort & Spa on Friday, September
12, 2008. In addition to lunch, awards and gifts will be given to the nominees.

Please use the following criteria and format to submit your Outstanding Ombudsman/Key
Volunteer(s) of your command. The form is due August 22, 2008 NO EXCEPTIONS. You may
mail (ASYMCA, Santo Road, San Diego, CA 92124), fax (858.751.5769) or e-mail
(pbarber@asymcasd.org) your form. An invitation will be sent to your individual once the criteria
have been met.

Criteria
An Ombudsman/Key Volunteer must contribute to their current command, maintaining effective
communications between the families and the command.

We will honor the following number of Ombudsmen/Key Volunteers according to the size of the
command. If you have more Ombudsmen/Key Volunteers that your command would like to honor
than the number designated for your command size, we will honor them if your command covers
the costs for the number of individuals beyond which you are designated. You must submit a form
for every Ombudsman/Key Volunteer you wish to recognize. The cost is $35.00 for each additional
volunteer, which covers his or her lunch and recognition items.
