DONATIONS!
The 2011 EHS Senior Celebration is right around the corner and we are now collecting specific
donations to be used as prizes the night of the celebration.

At this time we are requesting donations in the form of:

Cash
Gift Cards (for your senior’s favorite place or item)
Checks (made out to EHS PTO/ Class of 2011 - note Senior Celebration in memo line)

Please either mail your items to EHS, Attn: Senior Celebration Donations, 4525 Hwy 109, Eureka, MO 63025
or drop them off at the Main Office, Attn: Julia Jones

Ticket Purchase & Permission Slip

Senior Celebration Ticket Prices
May 23 - 24, 2011 $35 between January 15 and April 30, 2011
$40 after May 1, 2011
10:30pm to 5:00am

EVERYONE MUST TURN IN THE PARENTAL PERMISSION SLIP BELOW TO BUY A TICKET

Doors will open at 10:30pm and will be locked at 11:15pm. No one will be allowed in after that time. If your child has not
arrived by 11:15pm, you will be called.

Senior Celebration Parental Permission Slip

My child _____________________________________________ has permission to attend Senior Celebration 2011 on

May 23, 10:30pm - May 24, 5:00am at Eureka High School. I understand that my child must be at school NO LATER than
11:15pm and WILL NOT be allowed to leave before 5:00am.

If your child has not arrived by 11:15pm, a Senior Celebration Committee Member will call you.
