The Board of Trustees of Minidoka County Joint School District No. 331 requires all school
personnel involved in observation, examination or evaluation of students not to disclose to any
person the name or address of a student with physical, mental, educational, or social handicaps,
nor data resulting from such observation, examination or evaluation, except where such
disclosure is authorized by school procedures or by written request of the student's parent or
guardian.

Provisions:

The following provisions are delineated:

1. Information in student personnel folders and in health records shall be regarded as


confidential.

2. All information deposited in the offices of the Minidoka County Joint School District No.
331 and the Director of Special Services shall be considered confidential.
