It is the policy of the Board of Trustees of Joint School District No. 331 Minidoka, Jerome,
Lincoln, and Cassia Counties that the legal parent, custodian or guardian of any child seeking
admission to attend grades preschool and kindergarten through grade twelve, shall provide a
statement to the principal or her/his designee of the school the child is to attend regarding the
child’s immunity to certain childhood diseases, as more specifically outlined in Section 16, Title
02, Chapter 15 of Idaho Administrative Procedures Act, entitled “Immunization Requirements
for Idaho School Children.”

Failure to comply with the immunization requirements established by the State as set forth herein
or to otherwise provide a statement regarding Exceptions or Exemptions which may apply, shall
result in the admission and/or attendance being denied.

Any child admitted to school and failing to continue the schedule of intended immunizations will
be denied continued attendance until documentation of the administration of the required
immunizations is provided to the school authorities by the child’s legal parent, custodian, or
guardian.

A copy of the State of Idaho Certificate of Exemption form is attached.
