PROMISSORY NOTE

FOR VALUE RECEIVED, I, ________________________________, of legal age,

Filipino, married and residing at Cantilan, Surigao del Sur, (BORROWER), promise to pay
DANTE E. TAN and/or MENIA E. TAN, both of legal ages, Filipinos, and likewise residing at
Cantilan, Surigao del Sur, (LENDERS) the sum of TWELVE THOUSAND FIVE HUNDRED
PESOS (Php12, 500.00), Philippine Currency, payable on or before December 31, 2011. I
likewise agree to pay the LENDERS an interest amounting to FOUR HUNDRED (Php400.00)
every week payable every Saturday starting March 19, 2011 and every Saturday thereafter until
the amount lent is fully paid.

In case of failure to pay on the due date, the LENDERS shall be entitled to recover 10%
penalty on the outstanding balance, including unpaid interest until fully paid. In such case, the
entire obligation shall become due and demandable.

In the event the LENDER shall engage the services of counsel to enforce its rights or
interests, the latter shall be entitled to recover from the BORROWER, as and for Attorney’s fee,
a sum equivalent to Twenty Percent (20%) of the outstanding obligation, inclusive of interest and
in no case be less than Five Thousand Pesos (Php5,000.00).
