Qualifications for Candidacy to the Student Executive Board:

Applicants must have the following qualifications:

1. A residence of at least 1 year in the HS department prior to the application.

2. Must have a general average of 80% or higher in ACADEMICS in the past school year.
3. Must have a conduct grade of S+ or higher in CONDUCT in all subjects in the past school year.
4. Must be willing, dedicated and committed to serve the Bedan Community
5. Must have the willingness and passion to work with different people
6. Must have no past and ongoing disciplinary cases with the Student Formation Office

Instructions for Application

o All Application forms are available at the Student Activities Office at the student’s lounge
o Each applicant should submit only ONE application form which must be accompanied by copies of the
following documents:

1. Photocopy of your 3rd year report card

2. Recommendation forms coming from:
 3rd year class adviser and
 Any subject teacher who knows the applicant well
3. Certificate of Good Moral Character from the Student Formation Office
4. Hand written essay on a yellow pad paper

o Place all of the above documents in a long brown envelope.

o The envelope must bear the applicant’s name at the upper most LEFT corner.

SUBMIT THIS APPLICATION FORM TO THE OFFICE OF THE VICE PRINCIPAL FOR STUDENT
ACTIVITIES ON OR BEFORE
