Past Health History Patient R.C.

23 years old female seldom get sick, though it was the second time that she was admitted. Year 2002 she was admitted at San Pedro Hospital under the service of doctor Feliciano due to LBM and Urinary Tack Infection, she could not recall what were the procedures done to her though her mother mentioned that she was given antibiotics to treat Mrs. R.C.s Urinary Tract Infection. Her mother also mentioned that her daughter is a fully immunized child.

Aside from her present condition she got the common sickness like cough, runny nose and fever, but she manage it with over the counter drugs like, Solmux for cough, neozep or decolgen for runny nose, and paracetamol for fever. She was influenced by her friend a pharmacist to take myra e thinking that the drug would make her look young. Though she eats anything but she like junk foods like piatos and cheese flavored processed foods. She also mentioned that she eat 6 times per day including snacks.

During her grade school she was an athlete. She is a track and field player specifically a field runner. She used to play inter school competitions and national competitions as well, like; Davao Regional Athletic Association Meet the old Southern Mindanao Regional Athletic Association Meet, and Palarong Pambansa. But right after she graduated in elementary she stops being an athlete. Since then the only form of exercise the she do is cleaning the house, and walking around their village.

During her teenage life she admitted that she is a beer drinker, she drinks hard beverages like tanduay, and emperador as influenced by her peers. Though when ask how often does she drinks she mentioned that she only drinks whenever there is an occasion like birthdays.

Right after she got married and be a mother she stops smoking and drinking. But craving for junk foods and less intake of water is still there. She also mentioned that she loves to drink soft drinks like coke and mountain dew every 12 in the afternoon since the climate on that time is hot.

Her husband Mr. J.C. a panatalan supervisor, is religiously taking multivitamins and stress tab. Thus she was influenced to do the same thing as well. She was encouraged by her mother in law to increase her oral fluid intake since her mother in law notice that Mrs. R.C. seldom drinks water, and admits that she only drinks water every time she remember to do so.

Present Health History

She was twelve years old and playing for Davao Colleges Athletic Association Meet as a track and field runner when she was accidently hit by his co athlete at left breast during a game. She did felt pain after the incident but she takes it for granted, thinking that it would not affect her.

When Patient R.C. was 18 years old taking a bath before going to school she notice that there is a mass growing in her left breast. After taking a bath she told her mother with regards to what she had discovered. Mrs. R.C.s mother said that maybe it was just a part of the normal changes during menstruation and it would disappear if frequent soft massage would be applied. Until she noticed that it becomes big.

Mrs. R.C. mentioned that the moment she discover that the mass in her left breast gets bigger she tries to manage it by taking food supplements and other drugs promoted on television like MX3 and Skwelin. She was encouraged by the effects of the drugs on the patient that was endorsing the said supplements. But according to her nothing has changed. There is no consultation done.

One year after Mrs. R.C. decided to be examined by the doctor. She submits herself for check up at Limso Hospital under the service of Dr. Tan. She said that Dr. Tan told them that the mass on his left breast is still manageable with oral drugs that have dissolving effects. She could not recall what drug it is but she mentioned that Dr. Tan order her to take the drug for one month and return for check up after a month. But Mrs. R.C. was not able to have a followed up consultation to Dr. Tan, since according to her she was not able to complete the treatment regimen.

When Mrs. R.C. turns 20 she was scheduled for removal of the mass on her left breast at Southern Philippines Medical Center under the service of Doctor Inigo. But they found out that Mrs. R.C. was 3 weeks pregnant. Thus the operation was no done.

August 6 this year Mrs. R.C. is scheduled for operation for biopsy of the breast mass at Magsaysay Mission Hospital at General Santos City but since Credit Intelicare an insurance card was not applicable in the said hospital the biopsy was not done. The following day at Southern Philippine Medical Center Mrs. R.C was scheduled for stat biopsy but since they have to pay 5000 pesos as a downpayment for the procedure and Credit Intelicare is not applicable still the procedure was not done.

August 9, 2011 Mrs. R.C. and her family decided to have a consultation check up at San Pedro Hospital under Doctor Incarnacion. Ultrasound was done and on August 11, 2011 patient R.C. was admitted since the insurances are accept in the said hospital.
