Actor Contract

I, _____________________, hereby give my consent to be filmed and photographed by Group 2 (consisting of James Reader, Frank Tan and Olivia Cole) for their A2 Music Video project at The Latymer School on the following dates: Tuesday 1st November Friday 4th November Monday 7th Friday 11th November

I also understand that I may not be needed for these days, and that there may be extra sessions added where I will need to attend. I also agree to the Conduct Code listed below: I will be prompt to the shoot at the times given by the members of the Group If there is a problem meaning I may not arrive at the given time, I will communicate with a member of the Group before the arranged meet time I will act professionally and behaviour in an acceptable manner whilst on set I will behave and keep myself safe whilst on set In return, my group will: Allow me regular breaks throughout the day Provide refreshments to meet my catering needs Understand and respect the time of shooting and any time constraints I may have Signed, _________________ (Actors Name) _________________ (James Reader) _________________ (Frank Tan) _______________ (Olivia Cole)
